SafeHope Home is a non-profit organization dedicated to supporting women who are survivors of human trafficking and sexual exploitation. The organization provides comprehensive long-term programs, including housing, counseling, and life-skills training, to help women rebuild their lives in a safe and supportive environment. SafeHope Home also offers outreach services to connect survivors with essential resources, such as healthcare, addiction support, and peer mentoring. Through advocacy, education, and hands-on support, SafeHope Home aims to empower survivors, helping them regain their independence and reintegrate into society with confidence and dignity.
**Job Title**: Community Outreach Support Worker
**Overall Purpose**:
**General Duties**:
- Follow risk management, participant protection policies, and ethical guidelines
- Maintain accurate documentation while ensuring privacy and confidentiality
- Collaborate with the team to solve problems and achieve goals
- Uphold the organization’s mandate
**Primary Duties**:
- Conduct monthly 1:1 case management meetings with assigned Outreach clients
- Develop curriculum and programs for the Outreach initiative
- Liaise with therapists, doctors, and addiction counselors to ensure continuity of care
- Support Outreach program development, including creating safety kits and procedures
- Facilitate monthly Outreach meetings, workshops, and programs, combining therapeutic and recreational activities
- Provide safety planning and lifestyle awareness information to clients
- Conduct weekly wellness checks on Outreach/Follow Care clients, offering support and referrals
- Participate in Outreach activities, including delivering practical needs and meeting clients in the community
- Facilitate referrals to healthcare, educational services, and other community resources
- Collect and analyze data to evaluate the success of programs and identify new needs
- Input client notes within 24 hours of shifts
- Track and celebrate client milestones (e.g., birthdays, sobriety anniversaries),
- Provide court support when necessary
- Collaborate with community partners, maintaining positive relationships
- Collect donations and assemble care packages for Outreach clients
- Provide in-person support to Follow Care clients to ease their transition to independence
- Perform other duties as required
- Travel as needed
**Qualifications**:
- Previous experience in case management, outreach, or community services
- Strong understanding of risk management, participant protection, and confidentiality protocols
- Excellent interpersonal skills and ability to work with diverse populations
- Ability to develop and implement programs and support services
- Strong organizational skills, with experience in record-keeping and data collection
- A valid driver’s license and ability to travel
**Job Types**: Full-time, Fixed term contract
Contract length: 7 months
Pay: $27.00-$30.00 per hour
Expected hours: 35 per week
