**Job Title**:Social Worker (System Navigator)

**Reports to**:Executive Director

**Location**: New Vision Family Health Team’s main site currently located at 421 Greenbrook Drive, Kitchener, Ontario, Canada.

The New Vision Family Health Team (NVFHT**) **is seeking a full-time (1.0 FTE), permanent, **_Social Worker (S_**_ystem Navigator)._**

The NVFHT is a multi-disciplinary team located in the Waterloo Region providing primary care to over 26,000 patients. We offer a friendly, collaborative and professional environment in a spacious facility.

**Position Summary**:
As an integral member of the NVFHT, the Social Worker will have thorough knowledge of the health care system as a whole - both local and at a system level. This individual will act as a resource for patients by triaging and facilitating care as it relates to mental health services. The role also requires a high level of interpersonal skills to interact with patients and their families.

**Major Duties and Responsibilities**:

- Provide system navigation for patients referred by the primary care physicians or other members of the health care team.
- Assist patients and their families in navigating the health care and/or social services system.
- Facilitate initial screening and provide resources when necessary.
- Identify situations that warrant urgent follow-up or referral to external crisis services.
- Provide referral information for other community resources/services as appropriate to the patient situation.
- Facilitate groups in response to specific needs or populations identified by the team.
- Work with existing community groups / organizations to coordinate the delivery of care.
- Advocate for patients who encounter difficulties in accessing health education programs and services.

**Qualifications**:

- Masters of Social Work (MSW) and registration with Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Minimum 3 years relevant experience (e.g., intake, navigation, crisis intervention, triage).
- Excellent clinical skills and experience with assessment, treatment and management of mental health problems commonly seen in primary care.
- Case management/system navigation skills and knowledge of community resources.

**Job requirements**:

- Availability for at least one evening per week.
- A Vulnerable Sector check. Proof of Covid-19 vaccination (2 shots) approved by Health Canada and any booster shots recommended by Health Canada.

**Compensation and Benefits**:

- Hourly rate $38.54 ($75,153 annually).
