**Position Summary**:
As a member of a multi-disciplinary, the inpatient specialized oncology Social Worker provides assessment, treatment, support, and service coordination to patients and their families throughout their hospital stay and assists with transition back to the community. The outpatient specialized oncology Social Worker provides assessment, treatment, support, and service coordination to patients and their families. This position plays an important role in symptom management and incorporates Ontario Health’s systems and tools including Patient Reported Outcome Measures into standardized workflow. Additional responsibilities include, actively participating in research and quality improvement initiatives throughout the Cancer Program and Ontario Health, being a contributing member of an oncology quality council(s), psychosocial oncology team. They also provide or facilitate care for patients with mental health concerns who often have unique challenges in accessing and receiving cancer treatments. Additional responsibilities include serving as an expert consultant to the interdisciplinary team.

**Position Requirements**:

- Masters of Social Work degree.
- Membership in the Ontario College of Social Workers and Social Services Workers.
- Working knowledge of eligibility requirements for municipal/provincial employment and income supports and how to access such services.
- Knowledge of formal and informal community services and resources.
- Working knowledge of the Mental Health Act, Health Care Consent Act, Child & Family Services Act and any other relevant legislation.
- Understanding of psycho-social functioning. Ability to diffuse conflict and foster collaborative working relationships.
- Demonstrated ability to organize, prioritize, problem solve and respond to competing, unscheduled demands.
- Experience and skill in complex decision making processes, advancing best practice initiatives, and exploration of innovative models of care.
- Excellent interpersonal, written and verbal communication, organization, and time management skills.
- Self-directed, organized, and diplomatic.
- Patient navigation focused.
- Demonstrated ability to function independently and as part of a multidisciplinary team.
- Experience with palliative care an asset.
- Experience with quality improvement an asset.
- Skilled at providing virtual (video and phone) care
- Proven experience in self-directed learning and peer evaluation processes.
- Good computer skills.Excellent attendance, punctuality, and work performance record.
