**about the role**:
The Case Manager (Youth Resource Worker) engages in a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet the comprehensive health needs of Black 2SLGBTQ youth and young adults 14-29.

**day to day responsibilities**:
**Case Management**
- Performs intake assessments, works with service users to identify goals and develops a smart plan that supports the service user to achieve their goals.
- Facilitates coordination, communication, and collaboration with service users, as well as internal and external partners to support service user goals.
- Determines risk factors for threats of harm to self and others, mental health issues and substance use and prioritizes support based on urgent needs.
- Provides brief supportive counselling to service users in distress, when necessary
- Facilitates and participates in case coordination meetings to facilitate service user access to care, as needed.
- Evaluates outcomes of plans and facilitates moving on process.
- Maintains records in the electronic medical recording system, in accordance with Sherbourne policies and procedures and privacy legislation.

**Workshop and Group Facilitation**
- Plans and organizes workshops and/or short-term group programming.
- Conducts outreach to targeted participants for workshops and short-term groups.

**Administration**
- Participates in team and organizational planning processes.
- Operates within established policies and procedures.
- Maintains service statistics.
- Participates in ongoing evaluation of programs and service delivery.
- Participates in Sherbourne meetings, committees, special events, etc.
- Performs administrative tasks as required.

**To thrive in this role, you’ll need**

**education/experience**:

- Post-secondary education in social work or equivalent formal training
- At least 3 years’ work experience with 2SLGBTQ youth and young adults from communities that experience systemic marginalization/ oppression or an equivalent blend of education and experience.
- Case Management Certificate from a recognized educational institution, and current registration with the Ontario College of Social Workers and Social Service Workers are an asset.
- Experience using Electronic Medical Recording systems, Accuro preferred.
