**Child and Youth Health Program Facilitator**
1 Part Time Position (17.5hrs/week)

2 Year Contract

**Reporting To**:Manager, Community Wellness & Engagement

**Department**: Community Wellness & Engagement

**Purpose**:
The Children and Youth program at SCHC has created a youth led health committee that comprises of youth who voluntarily meet on a weekly basis during the school year to develop their skills and capacity as well as plan programs, initiatives and events that promote the holistic health and wellbeing of children, youth, and their families in the community. The Child and Youth Health Program Facilitator will help plan and implement and lead in facilitating various health programs based on the Social Determinants of Health and the needs of children ages (6
- 12) and youth ages (13-24) using evidence informed practices.

**Key Areas of Responsibility**:
Program Activities:

- Plan, evaluate and lead in facilitation of various health and evidence based programming including: o Gender specific social support groups, recreational activities, cooking, youth leadership development, fitness training, dance, arts and crafts, mindfulness, health promotion on social media, photography and videography
- Developing workshops and materials that identify current issues that impact youth in surrounding communities such as healthy lifestyles, sexual health, employment equity, healthy relationships as well as promoting awareness of resources available in the community
- Work as part of the team in developing and promoting programs.
- Utilize an anti-oppressive framework to create an interactive, inclusive and supportive environment for everyone
- Support and work in collaboration with youth leaders, volunteers and students to deliver programs
- Provide day-to-day support and direction to youth, volunteers, and students
- Assist with food preparation, service and cleanup
- Support in executing event activities including event setup and take down
- Work in partnership with internal and external services to provide referrals etc.
- Participate in organizational and external committees and QI initiatives as required
- Maintain a safe and hygienic workplace with particular attention to cleaning and sanitizing of the physical environment and equipment according to organizational policies
- Adhere to all policies and procedures of SCHC and promote mission, vision and values

**Program Development**
- Assist with program participant recruitment and registration
- Provide supportive engagement to participants, parents, and community partners
- Help develop and manage social media campaigns on Twitter, Facebook, Instagram and other platforms
- Monitor and ensure safety both on site and in the community programs
- Co-design and contribute to developing health promotional resources for children/youth
- Work with youth to provide ongoing feedback in regard to effectiveness of the programs, development, direction and evaluation
- Conduct administrative duties as assigned (i.e. collecting and inputting statistical information).
- Conduct electronic documentation as per SCHC Documentation Policy
- Support administrative and evaluation components of the program by tracking and reporting key metric measurements

**Community Engagement & Outreach**
- Orchestrate promotion, outreach, and participant registration strategies
- Develop cohesive action plan outlining outreach strategies
- Assist in community outreach and awareness for all programs while achieving targets for participant registration and engagement
- Research and monitor youth sector social media activity while increasing online audience and outreach
- Establish partnerships with other agencies and stakeholders to enrich programming, training, and shared knowledge

**General**
- Work cooperatively in a team setting with staff, volunteers, and students
- Participate in regular training sessions and staff meetings
- Work with all team members and supervisors to ensure strong communications and a positive working environment
- Perform other duties as assigned that are appropriate within the scope of the job

**Key Requirements**
- Strong facilitation skills and experience in program development and delivery
- Health enthusiast and has a sound understanding of the social determinants of health
- Excellent interpersonal, communication, organizational and presentation skills
- Experience working with youth, residents and volunteers
- Ability to work in a diverse multi-cultural environment with experience working with diverse groups
