**Hours of Work**:37.5 weekly - 1p to 9p - Monday to Friday - On Call weekend every 4-6 weeks
Under the direct supervision of the Manager of Joan’s Place and the general supervision of the Director of Shelter, the Harm Reduction Program Coordinator is responsible for providing integrated case coordination and harm reduction services to women residing at Joan’s Place.
This role supports individuals with complex care needs, including mental health challenges, substance use, and housing instability—through trauma-informed and harm reduction approaches. The coordinator works collaboratively with shelter teammates, outreach teams, and community partners to ensure responsive, client-centered support and continuity of care.
**Responsibilities**:
- Shelter Support & Case Coordination_
- Maintain accurate and timely client records in HIFIS, in accordance with organizational policies and procedures
- Collaborate with Diversion and Outreach staff to ensure smooth operations and coordinated service delivery at Joan’s Place
- Provide regular updates on open files and emerging issues during weekly team meetings
- Foster positive, professional relationships with external agencies serving the same client population
- Participate in departmental training and continuous improvement initiatives within the Women’s Housing Department
- Facilitate harm reduction education and programming for clients and staff
- Maintain collaborative relationships with agencies focused on substance use, mental health, and trauma-informed care
- Develop cultural competency with a focus on Indigenous traditions and community engagement
- Work closely with other YWCA departments to ensure integrated support for women and families
- Administrative Duties_
Follow case planning processes and maintain detailed electronic files including:
- Intake/assessment documentation
- Person-centered case plans, goal setting, and regular home visits
- Follow-up assessments and check-ins
- Provide warm referrals to community agencies and programs
- Complete VI-SPDAT/SPDAT assessments
- Track referrals, contacts, and statistics in accordance with program standards
- Attend weekly meetings to coordinate caseload and ensure regular communication
- Facilitate and lead internal and external presentations as needed
**Qualifications**:
- Experience working with Indigenous communities and cultures
- Bachelor’s degree in Social Work, Justice Studies, or related human service field, or equivalent combination of education and experience
- Minimum 2 years of case management experience preferred
- Experience supporting individuals with complex needs and homelessness
- Knowledge of Child Welfare, Trauma-Informed Care, and Family Violence are assets
- Strong independent and team-based work skills
- Accurate record-keeping and basic computer proficiency (HIFIS experience is an asset)
- Training and facilitation experience
- Strong communication, organizational, and crisis intervention skills
- Cultural awareness and respect for diverse backgrounds
- Valid Class 5 driver’s license
- Valid First Aid/CPR certification

Pay: $23.90-$25.10 per hour
