Care and Housing Co-ordinator

Sanctum is a non-for-profit organization that is operates a HIV Transitional Care Home and Hospice. Sanctum’s vision is to provide a community for individuals requiring supportive care to attain their optimum level of health, while maximizing their control and dignity.
We are looking for a Full time Care & Housing Coordinator to work with our organization in meeting our mission of providing care to people living with HIV/AIDS that is dignified, non-judgmental and unconditional.
Main Activities
- Work with people living with or at high risk for HIV/AIDS who may also be affected by mental illness, addictions, poverty, homeless and other psycho social factors
- Locate housing and provide coordination with residents to secure housing for transition out of Sanctum
- Provide supportive care through a patient centered model aimed at creating independence and autonomy while fostering an environment of acceptance and understanding
- Develop and maintain working relationships with housing services in Saskatoon
- Work with patients on individual goals and care plans aimed at recovery and management of their illness(s)
- Provide therapeutic counselling/assistance to clients who may present with multiple issues affecting their ability to cope and manage in the community
- Advocate on behalf of clients for additional service and resources, as required
- Establish and maintain current, accurate, confidential files for each client
- Ensure outcomes are recorded, measured and documented in the form of written reports
- Assist clients during their move into permanent housing. Such assistance may include accessing rental assistance money, security deposit, donated furnishings, coordinating with movers, and coordinating transportation.
- Assist client’s on overcoming any barriers to housing and provide crisis intervention/prevention as need in order to maintain housing
- Organize time and resources based on the individual needs of clients
- Manage any unanticipated events or unstable situations
**Qualifications**:
- Extensive knowledge of community resources with an emphasis on housing services and how to access and advocate for these services on behalf of residents
- Ability to communicate with and maintain an effective working relationship with clients from diverse social, economic and cultural backgrounds
- Demonstrated knowledge of the determinants of health and their impact on health related behaviours that increase risk of developing chronic disease.
- Demonstrated ability in assisting patients towards action steps for their health
- Willingness to adapt to new concepts, techniques and best practices in the academic and clinical fields of chronic disease management, addictions and harm reduction
- An understanding of the issues related to HIV including harm reduction
- Conflict resolution and interpersonal skills
- Background working in Mental Health and Addictions
- Degree in social work, addictions or related field
