Mental Wellness Outreach Worker

SERDC Health is accredited by the Canadian Accreditation Council to provide exceptional 2nd level services to eight or more First Nations communities in Manitoba. The Community Outreach Worker will carry out the duties and activities in partnership with the Mental Wellness Team from a culturally strength-based approach. The elements of care include intake, service planning, coordination, liaison and referral to appropriate services and resources; they will also provide ongoing monitoring, assessments of needs. The Outreach worker will liaison individual’s access to services, while engaging and encouraging participation in wellness plans.
**Responsibilities**:
- Advocate and Liaison for those in need of specific services.
- Assist in case management from a client/family centered plans for wrap around services.
- Working closely with SERDC Mental Wellness Team and local community wellness teams and other resources.
- Encourage effective screening, assessment service planning, referrals, service coordination, on-going monitoring and reassessment of needs in the continuum of care services.
- Support and follow up individuals and their families.
- Liaison with the community members and resources within the community.
- Work with local community staff to assess needs.
- Ability to work with youth, families, community leaders.
- Collaborate with local mental wellness teams on strategies pertaining to Mental Health and Wellness.
- Provide One to One Trauma Support.
- Consultation with other Mental Health professionals such as Nurse, Physician, Psychologist/Psychiatrist in a collaborative practice with BF/BHC/NAYPS/NNADAP workers for integrated wellness planning.
- Case manage client wellness plans, connect with other supports and ensure an aftercare plan is in place.
- Service broker resources for individual, family and community.
- Risk/Crisis Management.
- Prepare reports as required, including statistical data for federal reporting
**Qualifications/Requirements**:
- Demonstrated success working with First Nations communities in related capacity.
- Applied Suicide Intervention Skills Certification.
- Critical Incident Stress Management Certification.
- Mental Health First Aid Certificate.
- First Aid/CPR Certificate.
- Life Skills Coaching Training would be an asset.
- Case management and the development of integrated family centered care plans.
- High level of organizational, administrative and facilitative skills
- Must have a valid driver’s license and willing to travel, with occasional overnight stays in communities.
- Ability to lift at least 50lbs and fly to remote northern First Nation communities
**Other Requirements**:
- Able to lift at least 50 lbs. and travel by all forms of aircraft and watercraft to remote and northern First Nation communities in Manitoba. Overnight may be a requirement on occasion.
