Job Summary
The Resident Service Coordinator participates in the Nursing process to provide quality care and services to residents and their families/SDM from the time of admission to discharge and/ or death.
The Resident Service Coordinator will coordinate the admission process in collaboration with the Nursing team and facilitates the transition from home/community to LTC home.
The Resident Service Coordinator provides support in the marketing of the Home, may participate in community marketing fairs and other related external functions and organizations as coordinated with the Executive Director.

Responsible to:

- Executive Director, Director of Nursing, & Director of Care

**Responsibilities**:

- Liaises with prospective clients, provides guided tour of the Home and information relating to care and services provided.
- May be required to conduct tours after regular hours and weekends.
- Prepared rooms for a new admission.
- Reviews and summarizes assessments provided by HCC Ontario and reviews them with the DOC for approval for admission.
- Participates in meeting of newly admitted residents and their families and completes required consents and other information - this may vary in each Home.
- May be requested to initiate 24 hour RCP as requested from admission assessments, and from family information.
- Provides initial MDC meeting date, attends and assists with documentation of conference notes as necessary. Works in collaboration with Charge Nurses in chairing MDC conferences. Provides follow up to ensure documentation is completed.
- Tracks the annual MDC’s and ensure completion of documentation—provides monthly calendar to DOC and units.
- Provides follow up with family & hospital when a resident has been admitted to hospital
- Provides follow up with family when residents are on vacation leave.
- Liaises with the DOC and Administrator to provide on going communication and follow up with families as needed/requested.
- Follow up with residents/families when issues/concerns have been brought forward
- Participates in the facility wide Infection Prevention and Control Program. Assists the ICP in providing infection control information to residents, families and other visitors.
- May participate in Family Council and/or general family meetings
- Participates in the facility wide CQI and Risk Management Program
- Performs other duties as assigned by the Director of Care & Director of Nursing & Administrator.

QUALIFICATIONS:
Formal:

- Social Service worker, Community Mental Health worker, RPN certificate from the College of Nurses of Ontario and is in good standing is preferred.
**Experience**:

- Demonstrated ability dealing with unusual situations; set priorities; deal with stress and time pressures.
- Minimum of 2-3 years in Long Term Care preferred

- Excellent interpersonal skills
- Effective verbal and listening communications skills; communicates clearly and concisely
- Effective written and oral communications skills
- Excellent time management skills
- Ability to work with people and to follow directions from others.
- Ability to work with other departments and to show flexibility in emergency situations.
- Willingness to follow company policies.
- Must be an effective team player that works well with others.
- Must be able to work and communicate effectively with long-term care Residents and families
