**The Opportunity**

We are looking for **Community Development Intensive Case Managers** to join our Housing Team at CUPS.

As a **Community Development Intensive Case Managers (CDICMs),**you will play a vital role in delivering compassionate, dignified, and trauma-informed care to clients as they work toward long-term housing stability. Through a strengths-based approach, CDICMs support individuals and families living in place-based housing by fostering housing and health stability, promoting skill development, and enhancing social and community connections. Working within CUPS’ Integrated Care service delivery model, CDICMs collaborate with both internal CUPS providers and external partners to build a holistic network of supports tailored to client-identified goals.

Community Development Intensive Case Managers (CDICMs) foster belonging and connection within Community Development buildings by facilitating peer support, encouraging participation in social and skill-building activities, and strengthening natural and community supports.

Each CDICM supports up to 15 heads of household, providing services to the entire family through in-home visits, community engagement, and Resource Room support. They coordinate referrals, lead service planning, and collaborate on goal setting with participants. While based in an assigned building, CDICMs provide support across other Community Development sites as needed.

In addition, this role includes oversight and day-to-day coordination and cleanliness of place-based resource rooms to ensure they are accessible, welcoming, and responsive to participant needs.

**LOCATION**: 1001 10 Ave SW, Calgary, and throughout the community at place-based residences
**REPORTS TO**: Community Development Team Lead
**CONTRACT** **TYPE**: Permanent

**SCHEDULE**: Monday to Friday, 37.5 hours/week

**STARTING** **ANNUAL SALARY RATES**: $55,906 to $58,673

**Key Responsibilities**:

- Ensure clients receive respectful, trauma-informed and integrated care by assisting the client where they are at**,** providing information, support, and resources both internal and external of CUPS as appropriate and available.
- Advocate for appropriate client health care and support in maintaining housing while teaching patients and their supporters navigation and self-advocacy skills.
- Operate in an integrated manner with CUPS internal departments continuously assessing client needs, gaps in service, care plan modifications and advocating for the voice and self-determination of the client.
- Facilitate a healthy and safe work environment through learning and enacting all CUPS health & safety protocols, while maintaining an active awareness of clients and colleagues.
- Promote a culture that reflects CUPS Purpose, ambition, philosophies and values.

**Organizational Alignment and Performance**:

- Provide holistic support to participants including but not exclusive to case conferences, attending appointments, home visits, creation of care plans, supporting life enrichment and community connection activities
- Manage a Resource Centre including hosting social and skill building programming provided by both internal and external stakeholders.
- Provide culturally aware support to CUPS clients, celebrating the diversity of clients and being aware of power dynamics of privilege and oppression that may be present
- Meet with clients within the Resource Centre and creating a hub of resource materials & basic needs essentials available to clients
- Promote community involvement within the building through new tenant welcoming/volunteer orientation and opportunities, dynamic program delivery, client events and soliciting client feedback.
- Assist clients in defining strengths and needs through CUPS standardized assessment, and track progress with the client via follow up assessments to celebrate achievements and determine further needs
- In collaboration with the client, develop a care plan (or update previous care plan) to meet the needs identified and connect the client to services internally and externally to achieve goals
