**The Opportunity**
We are looking for an **Intensive Case Manager** to join our **Compass Housing Team** at CUPS.
As the **Intensive Case Manager**, you will be responsible for delivering trauma-informed care management to clients with complex health and social needs. You will be a member of multidisciplinary team providing wrap-around supports which honour participants’ autonomy, choice, and strengths. You will support participants in setting and achieving the client-directed goals and support their transition to community and other housing when the participant feels ready.
You will work within CUPS primary client service models of integrated & trauma-informed care by operating in partnership with all other CUPS client services linking clients to appropriate supports both within CUPS and externally as needed

**LOCATION**: Onsite, CUPS - 1001 10 Ave SW, Calgary, Outreach within Calgary Area**REPORTS TO**: Team Lead, Housing
**CONTRACT TYPE**:1 Permanent and 1 5-month Term Position
**SCHEDULE**: Full-time, Mon to Fri, **37.5**hours per week
**STARTING SALARY RATE**:$55,906 to $58,673
**Key Responsibilities**
- Provide holistic support to Homes for Health participants including but not exclusive to case conferences, attending appointments, client transport, home visits, creation of care plans, supporting life enrichment and community connection activities
- Advocate for patient needs and guide multi-disciplinary care teams on effective and respectful approaches to working with clients living with the vulnerability of poverty or homelessness
- Perform duties both alone and with colleagues in a variety of settings including in private homes, shelter, on the street, in camps, in office and while transporting clients in personal vehicle
- Assess client needs and fit with Homes for Health mandate and service scope, reporting to Homes for Health Team Leader if client needs require alternate services
- Provide culturally aware support to CUPS clients, celebrating the diversity of clients and being aware of power dynamics of privilege and oppression that may be present
- Ensure all required data is entered accurately into organizational databases including CUPS Resiliency Tool Assessment information, CUPS program Surveys, appointment notes and referrals provided
- Develop and maintain current Calgary resource and referral knowledge to efficiently navigate and advocate for client needs
- Support participants in the navigation of crisis and conflicts with landlords when needed in coordination with Housing Locator
- Coordinate the flow of client information to internal and external client care team members for seamless continuation of service in alignment with CUPS Integrated Care service delivery model
**Leadership**
- Create an approachable, caring and trauma-informed environment for clients in each interaction
- Advocate for appropriate client health care, health assessments, and supports in obtaining and/or maintain housing while teaching patients and their supporters navigation and self-advocacy skills
- Facilitate a healthy and safe work environment through learning and enacting all CUPS health & safety protocols, while maintaining an active awareness of clients and colleagues
- Promote a culture that reflects CUPS vision, mission, philosophies, and values
**Stakeholder Relations**:
- Support an integrated and holistic approach to client care, health promotion and maintenance and basic needs security, working in collaboration with Homes for Health team members including Nursing, Addictions Counsellor, Graduation Navigator and Housing Liaison
- Work in collaboration with client and external agency staff to develop appropriate care plans advocating for the client’s voice, health and housing needs to be incorporated into planning
