**Case Manager**

**(Permanent, Full-time 40 hours per week)**

**Bargaining Unit Position**

The Case Manager position involves providing direct, personalized support to residents through client-centered intakes, assessments, and case planning. The role operates within a strengths-based, relationship-centered care philosophy, focusing on the resilience of youth, particularly with an understanding of equity issues affecting Black youth. The purpose of the Case Manager role is to coordinate and provide comprehensive case management services that address residents' needs across various life domains, including housing, employment, education, mental health, and substance use. This involves advocating for residents, ensuring access to relevant services, crisis intervention, and fostering a safe and supportive environment conducive to positive outcomes.

**Highlights of Your Role**

**Client Support and Case Management**:

- Conducts intakes, assessments, and case planning.
- Develops short and long-term housing goals.
- Ensures residents' participation in treatment goals and programs.
- Evaluates service effectiveness and outcomes.

**Direct Assistance and Crisis Response**:

- Provides practical assistance in various life skills areas.
- Coordinates access to relevant services and resources.
- Responds to crisis situations, including suicide prevention and safety planning.

**Team Support and Collaboration**:

- Facilitates resident case reviews and attends community meetings.
- Liaises with in-house service providers and ensures smooth transitions.
- Supports staff and contributes to joint service projects.

**Professional Development and Representation**:

- Participates in meetings, training, and committees.
- Represents the agency professionally with external stakeholders.
- Promotes an inclusive and diverse environment

**Documentation and Database Management**:

- Manages client information and documentation.
- Ensures adherence to SMIS and EMHWare guidelines and maintains appropriate records.
- Documents according to professional standards any significant interaction with residents, maintains YouthLink Values: Leadership Humanity Innovation Respect Excellence police reports, underage contact reports, child protection reports, safety plans and medical feedback forms. Ensure information regarding service delivery is logged and recorded into the client information database by the end of each shift. Understands the role of empowerment-based documentation and the power of how documentation creates stories about young people. Ensure safety and security of clients, guests, staff and property.
- Logs information into the client database regularly.

**Highlights of What You Bring**

**Education (degree/diploma/certifications)**
- Undergraduate Degree in Social Work, Social Services Worker Diploma, Degree/Diploma in Child and Youth Worker, or equivalent education and experience.
- Valid First Aid / CPR certification is an asset.
- Valid CPI/SMG certification is an asset.

**Experience**
- Minimum of 2 years' experience in case management, preferably in a residential, housing, outreach, or shelter setting.
- Proven ability to case manage and access appropriate community resources for transitional shelter youth and provide housing related support.
- Demonstrated experience and ability to coordinate case management services resulting in seamless service delivery for residents.

**Knowledge/Skill/Ability**
- Training in client-centered intakes, assessment, and case planning
- Strengths-based relationship-based care philosophy
