**ABOUT THE ROLE**
Reporting to the Manager, Specialized Geriatric Services (SGS), the Psychogeriatric Resource Consultant (PRC) provides consultation, education, and support to healthcare providers and care partners involved in the care of older adults experiencing neurocognitive disorders (such as dementia or delirium), mental health challenges, and responsive behaviors. The PRC works collaboratively with teams across long-term care homes, the Northwest Local Health Integration Network Home and Community Care (formerly the CCAC), hospitals, Ministry of Health-funded community support agencies, and acute and post-acute care settings, as well as with clients, families, and informal caregivers.

**WHAT YOU BRING**
Education/Experience:
- Post-secondary education in a health care field such as nursing social work psychometrist or other degree-based health care with masters preferred
- Registered or eligible for registration with the appropriate professional college
- Minimum two years' experience in psychogeriatrics preferred otherwise significant experience in geriatrics long-term care or community based seniors care is essential.
- Experience working directly with seniors' mental health issues including Alzheimer disease and related dementias in a variety of sectors.
- Experience in program development research and evaluation community development and education
- Experience with psychogeriatric screening assessment and development of intervention strategies is required
- Competence in the use of standard assessment tools
Skills/Abilities:
- Ability to communicate effectively and concisely both orally and in writing with the ability to make presentations and deliver training to a variety of audiences
- Ability to establish and maintain effective relationships with superiors employees residents families and community partners ministries and other external stakeholders
- Proven ability to function and contribute as an effective team member and to work independently
- Excellent project/time management analytical/critical thinking problem-solving and decision-making skills
- Strong leadership skills including the ability to share skills and knowledge with others
- Must be proficient in the use of computers and current related word processing spreadsheet and presentation and health & safety software
- Must be competent within the meaning of the Occupational Health and Safety Act
- Demonstrated regular attendance at work
Assets:
- Experience with the P.I.E.C.E.S. program would be an asset
- Knowledge of the consultation process would be an asset
- The ability to converse in a second language would be an asset
Conditions of Employment:
- This position shall be conditional upon a satisfactory **Police Vulnerable Sector Check (PVSC)**where indicated, to ensure the absence of relevant criminal convictions.
- Will be required to travel to various work locations and provide own transportation
