**About the Opportunity**:
Reporting to the Primary Health Care Manager or Delegate, the incumbent will provide expert knowledge and skills on a wide range of issues from advocacy to assessment to case management. As a member of a primary health care (PHC) Diabetic education Centre team, the PHC Dietitian provides a range of PHC services. These services include, but may not be limited to, health promotion, illness prevention, screening, treatment and monitoring of people living with diabetes. Expertise may also be required to support people living with other chronic health problems, community development and advocacy. The incumbent will be required to collaborate with others to increase community capacity as an educator, researcher and clinical practitioner. These services are provided within a holistic, client/family centred care framework.
The incumbent will provide health assessment, interventions and appropriate follow up in accordance with acts, regulations, policies, standards, guidelines, mission and objectives of the Registered Dietitians of Canada, the Nova Scotia Dietetic Association, the Department of Health and Wellness - Province of Nova Scotia and the Nova Scotia Health.
**About You**:
- Bachelor of Science in Human Nutrition
- Completion of Dietetic Internship accredited by Dietitians of Canada
- Licensure by Nova Scotia Dietetic Association required
- 3 years recent related experience required working in a Primary Health Care setting/community-based team/community setting and experience with people living with a chronic condition
- Certified Diabetes Educator (or required to obtain within two years)
- Willing to complete requirements for necessary care directives
- Experience with individual and group education/facilitation utilizing adult education, self-management supports, and motivational enhancement principles
- Experience using virtual platforms such as Zoom and/or Skype to engage with patients and colleagues
- Preferred experience in partnership development; stakeholder collaboration, and community development
- Understanding of population health, health promotion strategies, and behavior change principles
- In-depth knowledge of local health network and community-based service
- Demonstrates person-centered / family-focused, diversity and culturally relevant approaches to care
- Strong interpersonal, teamwork, oral and written communication skills
- Demonstrates flexibility, adaptability and willingness to change
- Demonstrates innovation in program planning, implementation and evaluation, and quality improvement, planning skills
- Mandatory LMS modules as required
- Valid driver's license and access to a reliable car for travel for work
- Competencies in other languages preferred; French an asset

**Compensation and Incentives**:
$32.30 - $42.58 Hourly
