**Qualifications**
- Diploma in social services, gerontology or degree in Social Work.
- Registered under the Social Work and Social Services Work Act and holds a current general certificate of registration.
- Previous experience in a geriatric social related field is an asset.
- Well-developed interpersonal skills and demonstrated ability to interact efficiently and courteously with residents, public, employees and volunteers.
- Excellent written and verbal communication skills.
- Good organizational skills.
- Prior experience with clients in long term care and the community.
- Current First Aid and CPR certification
- Training, or the ability to obtain training in, P.I.E.C.E.S., Gentle Persuasive Approach, U-First, and Montessori
- Coordinates new admissions and repatriations from referring facilities
- Clinical visits to assess potential clients.
- Supports and facilitates patient flow directed discussion
- Collaborates with community partners to develop discharge plans and transitions to community/home
- Provides administrative and clinical leadership to staff in a supportive, facilitative and empowering manner
- Ensures appropriate adherence to the bed management contingency/ full capacity protocol and match patient needs with the appropriate environment and clinical support
- Participates in team building and promotion of good morale within the teams
- Provides supervision, direction, support, guidance, and/or education to clinical staff, as needed and in accordance with the policies, procedures, and guidelines established by Directors/Managers of each area
- Identifies areas requiring quality improvement; supports the process for identification of quality improvement initiatives related to patient flow, raises these issues at appropriate forums with recommendations for resolution
- Accountable for following standard work processes of the patient flow coordinator
- Engages in situational management and conflict resolution strategies
- Ensures that all situations handled are documented and forwarded to the appropriate Director/Manager
- Supports and assists in the implementation of all strategic priorities
- Coordinate and assist in cultural & timely inpatient admission and post-discharge authorizations, follow-up in transition of care coordination
- Respond to provider, member, and staff inquiries at any given time in a professional and timely manner.
- Collaborates with and provides information to patients, families, physicians, and staff regarding the provisions of care
