**Our Home Monitoring Program** enables individuals to maintain their independence through the help of our Licensed Practical Nurses (LPNs) and Care Aides who administer the medications in the patient’s residence (ADL’s, transferring or personal care is not within the scope of this position).

**Position Summary**

The Care Aide will be responsible for the care of all assigned patients within a specified routes(s). This responsibility includes the pickup, delivery, witnessing and administration of medication to patients within the scope of care aide practice at the location of patient’s residence. Through this support, the Care Aide allows individuals to maintain their independence in the community while ensuring continued care. ADL’s, transferring or personal care is not within the scope of this position. The Incumbent should have access to a personal vehicle.

**Skills and Responsibilities**
- Providing direct patient care and visiting patients’ homes
- Ensuring compliance with medications
- Actively participate in patients’ healthcare outcomes
- Witnessing medication where appropriate
- Acting in acute situations according to pre-set company protocols
- Documentation of all interventions, patient behavior changes and assessment data
- Submitting daily reports to Clinical Services Managers and pharmacies
- Screening of patients and notifying Home Monitoring team of any areas of concern
- Reporting missed dosage immediately for prompt resolution

**Position Requirements**:

- Must be BC Care Aide & Community Health Worker Registered
- Driver's License
- Access to personal vehicle
- Criminal Record Check will be requested upon successful offer
