Bayshore’s Integrated Care Solutions is seeking a Manager of Clinical Practice to lead our Bundled Care Program, supporting clients and caregivers through high-quality, community-based care. In this leadership role, you’ll collaborate with interdisciplinary teams, mentor frontline staff, and ensure care delivery meets the highest standards of excellence, compassion, and reliability.
What You’ll Do
- Lead holistic care delivery with nursing, PSWs, allied health professionals, and social workers.
- Partner with hospitals, HCCSS, and community organizations to ensure safe transitions.
- Mentor and manage PSWs and nurses, fostering growth and accountability.
- Ensure compliance with federal/provincial standards and Bayshore policies.
- Conduct case reviews and monitor client outcomes.
- Design individualized care plans for safe discharges and continuity of care.
- Address patient/family concerns with empathy and professionalism.
- Support staff development through training-focused home visits.
- Analyze intake processes and improve service access and efficiency.
- Oversee client assessments and ensure accurate documentation.
- Collaborate on program metrics and escalate concerns as needed.
- Participate in case conferences, team meetings, and professional development.
- Be available for flexible scheduling and on-call rotation.
**Qualifications**:
- What You Bring
- 5+ years in healthcare, ideally in home care and hospital settings.
- Registered Practical Nurse (Ontario) or Registered Social Worker with current CPR certification.
- Strong knowledge of community nursing, discharge planning, and Ontario’s health system.
- Proven leadership and mentorship skills.
- Familiarity with RAI tools (an asset).
- Valid driver’s license and reliable vehicle.
