Main Street Project (MSP) is a community health Centre with a mandate to address the social determinants of health which give rise to issues of addiction, physical and mental health issues and homelessness. MSP uses housing-first and harm reduction principles in the provision of a safe, respectful and accessible place for individuals who are homeless or at risk of homelessness in the community. Since 1972, the organization has been providing a range of services to Winnipeg's most marginalized residents, including emergency shelter and nutrition services, drug and alcohol withdrawal management services, health advocacy and support, case management services, housing support services, food bank services and a mobile community outreach program.

**JOB SUMMARY**
The Intake Worker is a key connection to potential and current participants of the Main Street Project's Withdrawal Management Program. The Intake Worker will connect individuals attempting to access MSP services and assist with identifying bed dates, building a connection, provide access to resources, and set a participant up for success within our WMS programs.
The Intake Worker will work with WMS Case Managers to assist with connecting to primary care, income, addictions & mental health supports to break down barriers and advocate for their participants. Following the completion of a participants stay in WMS services, the Intake Worker will continue to make contact with discharged participants to maintain connection and provide outgoing support. Relationship building is the key function of this role.
**KEY RESPONSIBILITIES**
- Bed Intake Monitoring: Monitor bed availability and intakes at two locations on a daily basis, ensuring timely and appropriate placement for individuals seeking withdrawal management services.
- Client Outreach: Routinely connect with potential intakes to assess their needs, provide information about the program, and offer support throughout the admission process.
- Post-Treatment Follow Up: Conduct post-treatment follow-up with Participants to assess their progress, address any concerns, and provide additional resources as needed to support their recovery journey.
- Assistance with Transports: Facilitate transportation arrangements for Participants as required, coordinating with relevant parties to ensure safe and smooth transitions to and from the treatment facilities.
- Collaborative Treatment Planning: Work closely with other WMS Case Workers to develop personalized treatment plans for participants; all unique needs and circumstances should be considered and reviewed
- Support the connection and work of Indigenous Relations and WMS Skabes within each program area and encourage participation in cultural programming through a lens of Healing and Harm Reduction
- Create and maintain a professional relationship with Community Members, including boundary setting, clear and concise messaging, and up to date documentation detailing interactions
- Keep accurate, continuous, legible and signed records including progress notes, checklists, correspondence, journals for individual and collective access in accordance with established protocols pertaining to access to and disclosure of WMS operational files
- Work with continuous care and consideration taking the power differential between the Intake Worker and participant into account at all times.
