One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking a Contract Outreach Coordinator for The Expand Project (November 2024-March 2025) as a valuable member of the Community Health Centre team. The contract will be extended on a part-time basis from April 1- December 31, 2025. The Expand Project is starting and expanding conversations around commercial and traditional uses of tobacco amongst 2SLGBTQIA+ young adults between ages 18-29. The focus will be on promoting The Expand Project during the pilot phase of the project and recruiting and leading advisory members to help with community engagement and outreach strategies.
**QUALIFICATIONS**:
- Bachelor’s degree in health and social service related field ( nursing, social work, gender studies).
- Minimum of 2 years' experience in a healthcare, social services, or community development role.
- Preference to those who identify as members of queer and trans communities.
- Familiarity with local community resources and services in Thunder Bay.
- Understanding of social determinants of health and health promotion principles.
- Comfortable using electronic medical record (EMR) and data management systems.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work both independently and collaboratively with an inter-professional team.
- Awareness and sensitivity to socio-economic and cultural issues.
- Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured).
- Valid driver’s license, access to a vehicle and ability to travel to other service locations.
