**Rate of Pay:$43.910 to to $54.895 per hour**

Reporting to the Manager - Regional Rehabilitative Care Program (RRCP), the Project Coordinator will be responsible for implementing clinical pathways for Community-based Stroke Rehabilitation for communities east of Thunder Bay in the Noojmawing Sookatagaing Ontario Health Team, and remote First Nations in the Kiiwetinoong Healing Waters Ontario Health Team. The Project Coordinator will collaborate with system stakeholders to assist with the implementation of the overall project goals, including engagement and evaluation.

The Community-based Stroke Outreach team provides specialized stroke rehabilitation services to underserviced rural and remote communities in Northwestern of Ontario. The program consists of a specialized stroke outreach team, including a physiotherapist, and occupational therapist and a social worker as well as a Stroke Navigator for the entire NW region. The project also includes education for the Outreach team and other community-based Stroke Rehabilitation teams in the NW region

**Qualification**:
Education/Experience:

- A regulated health care professional in good standing with their professional college with at least 5 years stroke specific experience in the Northwest region to understand impacts of stroke, the rehabilitation plan, and the stroke journey
- University degree in healthcare

Skills/Abilities:

- Clear understanding of the service and referral pathways across the region
- Ability to communicate effectively and concisely, both orally and in writing
- Strong project/time management, analytical/critical thinking, problem-solving, and decision-making skills

Assets:

- Working knowledge of stroke systems of care and stroke recovery as experienced by a stroke survivor or caregiver of a person with stroke
- Related experience working with stroke survivors in the acute, rehabilitation and/or community care aspect of the healthcare continuum
- An understanding of the uniqueness of the region and knowledge of Indigenous culture and health system challenges

**CONDITIONS OF EMPLOYMENT**:
This position shall be conditional upon a satisfactory Police Vulnerable Sector Check (PVSC) where indicated, to ensure the absence of relevant criminal convictions.
- Will be required to travel to various work locations throughout Northwestern Ontario and provide own reliable transportation
