The Rexdale Community Health Centre (RCHC) is a multi-service centre providing primary health care, health promotion and community support services, including settlement and community development activities for families, children, youth, adults and seniors of diverse backgrounds.
Rexdale Community Health Centre is expanding available services for mental health & addictions through partnership with the Centre for Addictions and Mental Health-CAMH and its Substance Abuse Program for African Canadian & Caribbean Youth (SAPACCY) and the Canadian Mental Health Association-CMHA’s Northwest Community Crisis Support Service (NW CCSS).
The Substance Abuse Program for African Canadian and Caribbean Youth (SAPACCY) provides culturally responsive services and care to youth (between ages 14 and 29) from the Black community. SAPACCY’s model is rooted in holistic, healing centered, Afrocentric, and culturally responsive care. Offering a range of prevention, early intervention, harm reduction, and recovery-oriented services, to assist youth and their families/caregivers in reducing harm, moving toward recovery, and making healthy choices for themselves and their family.
In addition, this position will support Northwest Community Crisis Support Service (NW CCSS). This service provides 24/7 support to people experiencing a mental health and/or addiction crisis in northwest Toronto. This pilot program is part of a City of Toronto police reform initiative that is evaluating the effectiveness and impact of non-police-led responses to people experiencing non-emergency crises. This program aims to reduce the harms experienced by people who have mental health and addictions challenges as well as Black, Indigenous, racialized, LGBTQ2+ and other marginalized individuals.
This position will work with a team of Mental Health Therapist and Case Managers who will be providing support to community residents facing non urgent crisis and Black youth/clients, as appropriate their families, who are dealing with mental illness and/or co-morbid addiction and as necessary, provide consultation and referrals to other providers serving this population. The successful incumbent will support the team to deliver care within the SAPACCY and NW CCSS models
**Responsibilities**:
- Track, monitor and report on all key project deliverables
- Cultivate relationships with the partners, stakeholder, youth and their families
- Support in the development of program materials and guidelines where required
- Implement risk mitigation framework, safety protocols and safety planning
- Provide oversight, support program staff
- Assist to inform the development of a case management system, outreach plan and referral processes/pathways for cohort participants
- Support in monitoring and building referral pathways for families for mental health, school supports with program staff
- Assist and support in outreach and social engagement with community, youth and their families on all social media platforms
- Liaise with all stakeholders and partners on matters that impact the implementation and delivery of program
- Use an anti-oppressive and Afrocentric practice
- In collaboration with the SAPACCY & NW CCSS Case Managers, develop working relationships and rapport with community service providers in providing culturally adaptive language & services which take into account the experiences of the African, Caribbean, and Black (ACB) community around racial trauma, racial gas lighting
- Support in intake and admin responsibilities
- Support team with linking clients and family members to supports and services that will promote their well-being, enhance their ability to meet their mental health needs and which aid in the exhibition of adaptive behaviours that address problematic substance
- Support staff to identify and coordinate community services to support youth and families’ mental health needs within their daily environment including school, home, employment, legal, and other community domains.
- Collect data relevant to program evaluation and quality improvement
- Coordinate and supervise the activities of SAPACCY and NW CCSS
- Conduct orientation and training on policies and procedures to all team members
- Be a positive influence, leader and coach of special projects team members
- Provide supervision sessions with team members to maintain integrity in
- service delivery
- Provide the Program Manager with regular reports regarding the strengths and challenges faced by program participants and with the strategies being used to address these challenges.
- Work with the Program Manager to ensure that information needed for program monitoring, evaluation and improvement is available and current, including collection of accurate and timely data on student attendance, grades and program placement.
