**Temporary Full Time **Transition Navigator**
Grand Erie District School Board represents more than 28,000 students in 58 elementary schools and 14 secondary schools within the City of Brantford and the counties of Brant, Haldimand and Norfolk, as well as students from Six Nations of the Grand River First Nation and Mississaugas of the Credit First Nation. **RESPONSIBILITIES**:
Reporting to the Principal Leader for Specialized Services and working closely with other system portfolios, school teams, local post-secondary schools and provincial associations, the Transition Navigator will support students with disabilities and their families through critical educational and life transitions. This role focuses on facilitating smooth transitions from grade 8 to grade 9, as well as preparing students for post-secondary life, including employment, further education, and independent living.
Key responsibilities include but are not limited to:
- Review and assess current practices for transitions of students with disabilities to post-secondary, as well as new to Grand Erie secondary school students with disabilities
- Engage with current and past students and families to gather feedback on their experiences with transitions exiting from secondary school
- Design a variety of resources to support school exit pathways and post-secondary transitions
- Provide personalized support and guidance to schools and families throughout the transition process
- Establish and maintain relationships with post-secondary educational institutions and community organizations that support post-secondary educational and employment transitions for people with disabilities
- Facilitate connections between students, families, and relevant post-secondary institutions and community services
- Design and implement programs and events to support families with:
- Connecting with supports through Passport funding for employment or education opportunities
- Transitioning to adult services
- Collaborate with school staff to ensure post-secondary goals are appropriately aligned and incorporated into students' program and classes
- Stay current with best practices in transition planning for students with disabilities
- Collaborate with Student Success and attend organizational and other departmental meetings when necessary
**QUALIFICATIONS AND EXPERIENCE**:
- A post-secondary bachelor's degree with a minimum of 3 years' experience in a leadership role in corporate communications/community engagement within education or the private sector.
- Demonstrated experience working with people with disabilities.
- Demonstrated core beliefs of inclusive practice including the areas of Anti-Racism, Anti-Oppression.
- Demonstrated awareness of Ontario Human Rights Commission Policy: Accessible education for students with disabilities; AODA Draft Education Standards K-12, Learning for All.
- Demonstrated awareness of the Ministry of Education PPM 149 and PPM 156
- Experience building trusting and collaborative relationships with a variety of partners
- Strong understanding of community resources and support services for individuals with disabilities
- Excellent verbal and written communication skills and interpersonal skills
- Strong organizational and project management skills

- Hiring Range $76,754 to $84,604
Salary Range $76,754 to $87,221
Eligible for OMERS pension plan
