South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Health Promoter - Vaccine Promotion & Education position works as part of the multi-disciplinary Vaccine Planning group. This position will lead and will work collaboratively with other team members, service providers and programs as well as Community Ambassadors to facilitate education, promotion, implementation of vaccine clinics. This position will lead work with partners and cross organizationally on development/coordination of webinars, community vaccine clinics, digital content for website and social media platforms, securement of speakers and sharing vaccine information presentation. This position will lead on collecting socio-demographic information and monitoring and tracking outcomes including ongoing project evaluation.
This position is expected to work onsite during the COVID-19 pandemic as required.
**Responsibilities**:
- Works collaboratively with appropriate community resources to develop and execute tasks related to the Vaccine (COVID-19) Promotion Project (outreach, digital promotion, develop/coordinate/delivery of public education, partnerships, vaccine clinic coordination/support)
- Works with Program Leaders to facilitate outreach and recruitment of participants for the project
- Facilitates and supports development of presentations, identifies suitable session presenters, facilitates successful delivery of vaccine education; provides support to presenters, community ambassadors and participants during webinars and program presentations
- Assists with the development of COVID-19 vaccine education sessions and digital content
- Keeps record of attendance, enters relevant data into the EMR, participates in ongoing project evaluation, social analytics and reporting
- Connects program participants to testing, assessment centres, treatment resources and vaccine clinics
- Engages with service users and program participants, providing vaccine education, materials and counselling related to COVID-19 boosters, spikevax and child vaccines
- Provides information and referrals to health and social service supports
- Prepares statistical data, digital content, presentations and other documentation, as required
- Participates in Communications planning, in collaboration with staff, other programs, management and partners
- Acts as liaison between community and healthcare providers to meet community and client needs
- Other duties as required
**Qualifications**:
- Post-graduate degree from a recognized university in social work, or a related discipline, or a combination of undergraduate degree and significant experience
- 3-5 years health promotion/education experience in a community-based organization
- Minimum 2 years’ experience working with marginalized populations
- Excellent group facilitation skills, experience and knowledge of health promotion and education, community organizing techniques and program design, implementation and evaluation
- Excellent coordination and organization skills
- Knowledge and experience with Digital Media Platforms (Social Media management software, WordPress, MailChimp)
- Experience in designing flyers (Canva or Adobe)
- Experience in community outreach, community education and engagement
- Demonstrated experience of community development approaches in high needs and marginalized communities
- Demonstrated commitment to and knowledge of community-based health care and community resources
- Proficiency in peer education strategies
- Strong oral and written communication skills
- Excellent interpersonal skills
- Demonstrated skills in partnership work
- Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
- Demonstrated ability to work effectively in a multi-disciplinary team environment
- Some project management experience
- Experience in working with our priority populations, including people living in poverty, those with diabetes and chronic disease, mental health and/or substance use issues as well as newcomers/immi
