We are funded by the Ministry of Health, Ontario Health, Ministry of Community, Children and Social Services, the City of Toronto and many generous donors and foundations.about the role:The Health Promoter plays a key role in enhancing the health and well-being of Sherbourne Health’s priority populations, with a particular focus on 2SLGBTQ communities. This position is responsible for planning, implementing, and evaluating community-driven health promotion initiatives that address the full spectrum of social determinants of health, including income security, employment, housing, and social inclusion. The Health Promoter supports all aspects of health promotion programming and leads targeted initiatives such as the annual Trans and Non-Binary career fair, employment peer support groups, and career mapping sessions. They also facilitate cohort-based and drop-in programs that are culturally safe, evidence-informed, and responsive to the diverse needs of the communities Sherbourne serves.scope of the Mandate:The Health Promoter will develop and implement targeted health promotion initiatives that are inclusive, evidence-based, and responsive to the specific needs of Sherbourne Health priority population (2SLGBTQ, newcomer, and Individuals experiencing homelessness). This role includes conducting community situational assessments, coordinating health education activities, and engaging in policy advocacy to address system barriers and promote equity in health and social outcomes.The Health Promoter will also engage in case management tasks, working collaboratively with service users to conduct assessments, establish clear goals, and coordinate care that considers the full spectrum of social determinants of health, particularly as they relate to employment, education, and access to safe and inclusive workplaces.Activities will be implemented at individual, community, and policy levels to increase access to equitable health services, reduce social and economic barriers, and support community-led initiatives that foster resilience and well-being. The Health Promoter will leverage research evidence, community feedback, and best practices to guide the development of programs, resources, consultations, and policy briefs.we believe in work-life development days per year – related to organizational objectives.Responsibilities and Tasks:Health Communications / Knowledge TranslationDevelop and disseminate culturally relevant health communication materials tailored to the needs of 2SLGBTQ communities.Translate complex health information and research findings into accessible information that can inform community action and policy advocacy.Act as a resource for program inquiries, provide accurate information and connect community members to appropriate services and supportsProgram Planning, Implementation, and EvaluationUtilizes an understanding of public health sciences – such as epidemiology, health promotion, disease prevention, and health protection – to inform and guide health initiatives for the TNB community.Conducts community-based research and situational assessments to identify health needs, barriers, and opportunities within 2SLGBTQ communities.Designs and plans for health promotion initiatives that are evidence-informed and tailored to the unique aspects of 2SLGBTQ health and well-being.Evaluates program outcomes using quantitative and qualitative methods, ensuring continuous improvement and responsiveness to community needs.Provides capacity-building training and consultations to community members, students and others on the needs and issues of 2SLGBTQ communities.Support the development and facilitation of Trans employment peer support groups and career mapping workshops, providing tools, mentorship, and resource navigation to reduce employment inequities and build economic empowerment among Trans and Non-Binary community membersProvides support to all aspects of weekly drop in programs, including outreach, program implementation, meal preparation and group facilitationOutreach, Engagement, and Partnership DevelopmentIdentifies and leverages existing community resources, to support the success and impact of the health promotion program.Develops and implements outreach strategies that increase engagement and participation in health promotion programs across 2SLGBTQ communities.Supports the development of partnerships with local community organizations, local health departments, schools, businesses, etc.Collaborates with partner organizations to share relevant information and resources, ensuring consistent communication and capacity-building.Case ManagementPerforms intake assessments, works with service users to identify goals and develops a smart plan that supports the service user to achieve their goals.Facilitates coordination, communication, and collaboration with service users, as well as internal and external partners to support service user goals.Maintains records in the electronic medical recording system, in accordance with Sherbourne policies and procedures and privacy legislation.AdministrationWorks with the Program Supervisor, Coordinators and Director to identify and prioritize issues, plan overall program goals and objectives and evaluate their effectiveness.Collaborates with team members to plan and implement Health Centre initiatives and interprets, implements, and supports established policies and procedures.Makes recommendations and contributes to policy formulation and quality improvement initiatives.Participates in the development, implementation and evaluation of programs and standards of practice for service delivery and accreditation.To thrive in this role, you’ll neededucation/ experience/ knowledge and skills:Education in public health, health promotion or health/social sciences at a Bachelor level or equivalent formal trainingMaster of Public Health is considered an assetDemonstrated knowledge of working with Sherbourne Health priority populations, specializing in trans and non-binary healthExcellent written and oral communication skills including writing for a variety of print and social media.Significant experience in group facilitation, community education and training.Good administrative and organizational skills including knowledge of computer software, project and event management and the production of narrative and statistical
