This client-centred case management approach includes: Creating client-centered case management plans, navigating relationships with natural supports, goal setting, crisis resolution, accessing addiction and/or mental health counseling, accessing financial support, basic needs, securing affordable and safe housing, cultural connections, systems navigation, finding and maintaining employment/education, and creating positive lifestyle opportunities.
