**TITLE**: Outreach Worker/TDSC Coordinator

**POSITION STATUS**:Part Time - Permanent (Non-Union)

**SCHEDULE**: Monday to Friday
- 35 hours per week (may be required to work an occasional evening, weekend or statutory holiday).

The Seniors’ Services - Outreach Worker will facilitate communication between Deaf Seniors and Hearing Consumers; to assist in the development and delivery of community outreach programs; to lead, coordinate and inspire TDSC Members to be active in the TDSC Community.

**PRIMARY DUTIES**
- Work as part of a multi-disciplinary team in identifying needs of TDSC Seniors while providing escort services
- Set up, coordinate and participate in all TDSC activities
- Monitor membership information and renewals; maintain membership database
- Promote active leadership within the TDSC, identify potential new members
- Attend monthly TDSC committee meetings; act as a resource and provide support to TDSC Committee members
- Coordinate and attend TDSC Annual General Meeting
- Networking and promoting the services and programs to the TDSC with other service agencies
- Assess, Evaluate and plan services and Participate in Organization activities i.e. Organization committee work
- To adhere to policies and procedures related to the transportation of consumers and the use of Agency vehicles, the completion of logs and schedule of Agency vehicles
- Ensure the safety of consumers while operating Agency or private vehicles. Reasonable care must be used when transporting any consumer. All problems and concerns must be reported immediately. The vehicle policy and procedures manual must be adhered to
- Adherence to all Organization policies and procedures including personnel manual, Occurrence Reporting Manual, Seniors’ Services Policy Book and Fire Manual
- Assist in the development of Community Outreach programs
- Collect and manage all TDSC trip deposits and payments
- Attend and participate in monthly Seniors Staff and Team meetings, and provide pertinent Client information
- Respond to emergency situations by following established guidelines i.e. fire alarm, Seniors Services section
- Collect monthly TDSC volunteer statistics and submit them to Volunteer Coordinator

**QUALIFICATIONS**
- Geriatric/ Social Services Diploma or equivalent
- Minimum 2 years’ experience in a Senior Care setting, preferably with Seniors
- Valid class G Driver’ License
- Fluent in American Sign Language and Two-Hand Manual Communication
- Knowledge and experience working within the deaf community and fluency in ASL **OR**strong commitment to engage in professional development to build skill and knowledge in ASL, Deaf culture and supporting Deaf or hard of hearing individuals and their community
- Excellent communication, literacy and interpersonal skills
- Knowledge and experience with report writing and data entry
- Demonstrated experience in computer literacy (e.g. Word, Excel, Outlook)
- French language/LSQ an asset
