The **Safe Space Coordinator **will oversee theBed Transition Program. This position works collaboratively with the Executive Director, Program Coordinator, Psychiatric Nurse, and Health Care Aides to develop a care plan that identifies and plans for a client’s immediate and long-term needs. This position will report to the Director of Operations.

**Responsibilities**:

- Screen incoming referrals from the Winnipeg Regional Health Authority and Shared Health to SBSL Bed Transition Program.
- Manage the intake process of referrals by collecting personal, medical, and social history.
- Provide case management for clients to transition to a recovery service, supportive housing, long-term care, or the Community Living Program, based on assessments for cognitive function and safety in the community.
- Make referrals to external supports regarding disability, home care, recovery services, and crisis services.
- Collaborate with client stakeholders and other service providers such as EIA, MB Housing, Pharmacy, Public & Guardian Trustee, Home Care, Community Mental Health, Doctors, and friends & family members.
- Coordinate transportation and supervision, dependent on level of independence, for community outings, appointments, hygiene needs, and personal laundry.
- Provide follow up support for clients in the Community Living Program.
- Complete applicable funding documents for EIA & treaty status clients who are eligible for medical equipment that has been requested by a certified health care professional.

**Qualifications**:

- Social Work Degree preferred; however, an acceptable combination of education, training and/or experience will be considered.
- Valid Class 5 Driver’s License.

**Skills**:

- Experience working with specific populations, such as individuals with mental health concerns and addictions.
- Experience with case management.
- Excellent interpersonal and communication skills
- Ability to work independently and as part of a team.
- Strong organizational and time management skills
- Commitment to ethical and culturally sensitive practice

Pay: $19.57-$39.65 per hour

Expected hours: 40 per week

Additional pay:
