**Job Description**: **POSITION PURPOSE** Reporting to the Program Manager, Community Programs & Partnerships, is responsible for analyzing program efficiency and effectiveness; recommending program improvements; ensuring responsive service delivery; making determinations for client appeals; and using available data to support program delivery and services that are legislated, under the corresponding Provincial and Regional Acts and regulations. **MAJOR RESPONSIBILITIES** - Waitlists management, co-ordinates admissions, case management, assessments and care planning and reporting activities, in compliance with all relevant regulations and legislation. - Ensures clients are provided with services that reflect the Branch’s commitment to a safe, secure and customer focused environment. - Reviews program statistical data necessary to ensure compliance and fiscal integrity with external funding partners, Long Term Care directives and Regional policy and procedures. - Ensures that services and programs are delivered to clients, the community and staff in accordance with appropriate legislation, service standards and policies; analyzes program efficiency and effectiveness. - Supervises client crisis situations involving immediate physical, emotional, social needs and becomes involved, as necessary. - Reviews legislation and provincial guidelines; implements Provincial and Regional programs, services and initiatives; recommends new practices or changes to existing practices and provides technical advice in the development of new practices. - Maintains critical awareness of evolving community and senior’s issues by participating in Community Programs planning activities. - Supervises staff, including recruitment, selection, hiring, scheduling, assigning and monitoring work, determining training and development needs, coaching and mentoring, conducting performance appraisals and determining/recommending disciplinary action up to and including dismissal in accordance with collective agreements, Regional policies and practices. - Promotes effective communication within the Division/Branch and encourages harmonious inter-professional relations among staff. - Assumes functions of the Program Manager in that person’s absence, as directed. **QUALIFICATIONS** - Successful completion of a University Degree in Health Sciences, Social Sciences, or a related program, or approved equivalent combination of education and experience. - Minimum three (3) years supervisory experience in a health/social service environment including direct casework, and service delivery. - Demonstrated supervisory and leadership experience. - Valid Class "G" Driver's License and reliable vehicle for use on corporate business. - Satisfactory Police Vulnerable Sector Checks (PVSC). - Demonstrated knowledge of relevant Standards, Acts and Regulations as they relate to community based care, Long Term Care, mental health and regulated health professionals. - Knowledge of Alzheimer Disease and other related dementias is an asset - Sound knowledge of the Department’s program delivery operations, programs and services, the community served and client social issues. - Knowledge of budget process and business planning. - Leadership competencies including operating strategically, leveraging diversity and differences, customer focused, ensuring accountability, managing complex problems, financial management, employing interactive and effective communication, fostering collaborative relationships, cultivating engagement to drive vision and purpose, managing and developing talent, establishing and maintaining trust, displaying self-awareness, and demonstrating resiliency. - Knowledge of and demonstrated ability in management competencies including operating strategically, leveraging diversity and differences, customer focused, ensuring accountability, managing complex problems, financial management, employing interactive and effective communication, fostering collaborative relationships, cultivating engagement to drive vision and purpose, managing and developing talent, establishing and maintaining trust, displaying self-awareness, and demonstrating resiliency. - Strong presentation, negotiation, mediation, problem solving, report writing, project management, research, analysis, and communication skills.
