**The Outreach Coordinator** will address the social and emotional needs of older adults through **assessment, support, and connecting to resources based on individualized case plans**. This position will work collaboratively with other workers and organizations to provide services, as well as identifying gaps in services and advocating for the needs of clients. **The goal of the program is to support the wellbeing of the senior and enhance their wellness in the long term.** This program is delivered with **commitment to equity, inclusion and honouring cultural and social diversities.**

**QUALIFICATIONS, SKILLS & EXPERIENCE REQUIRED**
- **Registered Social Worker with ACSW**:
- Relevant degree or diploma
- Assessment, interpersonal, problem-solving and decision-making skills required
- Self-motivated; able to work independently and as part of a team
- Computer literacy including Microsoft Office
- Proficient communication and collaboration skills
- Police Information Check including Vulnerable Sector is required
- Experience working with older adults and volunteers is an asset
- Experience working with diverse cultures is an asset
- Working knowledge of community resources
**Job Types**: Part-time, Fixed term contract
Contract length: 12 weeks
Pay: $23.32-$28.82 per hour
Expected hours: 28 per week
