Under the direction of the Chief Transformation Officer, VP Human Resources of Alexandra Hospital, Ingersoll and Tillsonburg District Memorial Hospital, and in close cooperation with the hospital's Leadership team, the Manager, Quality and Patient Safety will facilitate and guide the hospital's overall quality mission and related strategic priorities. As a member of the Leadership team, they will lead and assume responsibility for the development, implementation and monitoring of legislated Quality plans (e.g. Quality Improvement Plan (QIP)), legislated Patient Safety requirements, Accreditation Canada requirements and will support the hospitals' enterprise risk management plan and overall policy infrastructure.

**Key Responsibilities**:
**Quality and Patient Safety**:

- Work in collaboration with leaders across the organizations to enhance patient care by providing leadership to quality initiatives and facilitating identification of opportunities to enhance care and service delivery, and patient safety across both organizations.
- Assumes responsibility for communication and education regarding quality, Accreditation and patient safety, and risk in the organization.
- Supports a culture of delivering excellent quality care and patient safety.
- Prevent future harm and improve patient and provider experience by initiating and overseeing proactive evaluation and redesign of systems to improve care processes (i.e. prospective and retrospective analysis).
- Presents informative and actionable reports to Leadership, Medical Advisory Committee (as needed), and Teams Members.
- Oversees and monitors the development and implementation of each hospital's annual quality improvement plan (QIP), and the annual patient safety plan.
- Acts as a liaison to Accreditation Canada. Coordinates and directs activities related to the hospital wide review/survey; including, compliance with all practice and policy requirements. Ensures that identified deficiencies are addressed, resolved and communicated to leadership and to Accreditation Canada.
- Facilitates the development of policies, procedures and standards and oversees the organization and management of hospital policy framework.
- Performs other duties as required.

**Systems Management**:

- Create an environment where team members feel they are part of the team and that their contributions make a difference.
- Participate in defining, understanding and modelling the values and culture of the organization.
- Participate in development, implementation and evaluation of a quality management system for the hospitals.
- Develop, implement and evaluate Standards of Care and Standards of practice as it pertains to quality and patient safety.
- Collaborate with and recommend policies and procedures for hospital committees, i.e. Emergency Management Plan, and Emergency Response Committee.
- Membership on Integrated leadership Council, Emergency Response Committee, Patient and Family Centred Care Committee, and other committees as assigned.
- Use information systems to receive and disseminate information, to ensure appropriate and efficient utilization of resources and to monitor impact on quality and patient safety.
- Identify needs, assist in the coordination of the delivery of health services but more importantly assess the quality of the services (looking for improvements) for patients/families with other stakeholders and with community agencies.

**Financial Management**:

- Plan, prepare and submit for approval capital, operating and staffing budgets for each area of responsibility.
- Monitor and control utilization, spending and budget allocations to ensure adequate space, staff, equipment and supplies to fulfill identified needs.

**Patient Quality of Care Responsibilities**:

- Identify quality of care concerns and work with the Chief Transformation Officer/VP Human Resources to address these concerns and ensure the best patient outcomes possible.
- Lead Quality Reviews to support patient or staff incident analysis.

**Continuous Quality Improvement**:

- Support the investigation and monitoring of incident reports, i.e. medication, patient, visitor, employee and unusual occurrences as well as trends in these reports and direct corrective actions where concerns are identified.
- Review feedback from patients and implement plans for improvement in areas identified to be a concern.
- Lead the creation, implementation and monitoring the Hospital's Quality Improvement Plan, including strategies for improvement, measurement targets to be achieved and ongoing monitoring of performance in these areas.
- Provide written reports and participate/provide verbal reports when required at the Quality, Risk and Patient Safety sub-committee of the Joint Board.
