**Description du poste**
- Conduct comprehensive assessments of speech, language, communication, and swallowing abilities
- Develop and implement individualized care plans that address identified needs and align with the patient’s goals and preferences
- Provide interventions to optimize swallowing function and manage dysphagia to ensure comfort and safety during oral intake
- Assess the impact of symptoms such as pain, dyspnea, nausea, anxiety, and depression on swallowing function and communication performance. Develop and implement tailored strategies to manage these symptoms and support optimal function and quality of life
- Effectively communicates the individual's functional status, including challenges related to communication, feeding, eating, drinking, and swallowing, as well as the likely progression of these conditions. This is done with accuracy and compassion, while respecting the person’s needs, preferences, and potential changes in their abilities
- Acts as a mediator, educator and advocate for the patient regarding initiating, withdrawing and withholding artificial hydration and nutrition
- Assess caregivers’ skills and need for training and support, to assist with safe swallowing techniques, and communication strategies
- Educate patients, families, and caregivers on communication strategies and swallowing management techniques
- Collaborate with interdisciplinary teams, including but not limited to physicians, nurse practitioners, nurses, personal support workers, social workers, speech therapists, occupational therapists, and physiotherapists
- Maintains accurate and timely documentation of patient assessments, treatment plans, and outcomes, ensuring adherence to ethical and legal standards

**Qualifications**
- Completion of a Master's degree from a Speech Language Pathology program
- Registered with the College of Audiologists and Speech Language Pathologists of Ontario
- Minimum 3-5 years of experience working as an Speech Language Pathologist
- Successful completion of any of the following is considered an asset:

- Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or
- Fundamentals of Palliative Care
- The Comprehensive Advanced Palliative Care Education (CAPCE)
- Experience working in a community setting
- Ability to work autonomously and as part of an integrated health care team, with an emphasis on collaborative care planning
- Valid Ontario driver's license and a reliable vehicle
- Proficiency in computer software programs (e.g., Microsoft Office) and virtual communication platforms (e.g., Microsoft Teams, Zoom, Go To Meeting)
