The Assessor Coordinator will assist the client and family to obtain a high quality of care by jointly developing, implementing and evaluating a plan of care that meets the needs of the client and their family/supporters as the client leaves the hospital to continue with care or to support the client to remain independent in the home setting.

**Human Resources Exemption**:No

**Experience**
- One year of home care experience preferred

**Education**
- Bachelor of Science in Nursing (BScN)

**Licenses**
- Valid Class 5 driver's license

**Other Education and Training**
- Practical Nursing diploma is acceptable
- Bachelor of Psychiatric Nursing is acceptable
- Psychiatric Nursing diploma is acceptable
- Nursing diploma is acceptable

**Competencies**
- Advanced - Communication skills
- Advanced - Interpersonal skills
- Advanced - Organizational skills

**Knowledge and Abilities**
- Knowledge of the RAI-HC assessment tool
- Ability to adapt to changing conditions

**Other Information**
- Travel required
- Preference to HSAS members as per the SAHO/HSAS Collective Agreement
- Registered/eligible for registration with the appropriate licensing body
- Access to a reliable vehicle
