About The OpportunityWorking as an integral part of the Yúustway Health and Wellness Department. Working in accordance with the Vision, Mission and Values, and strategic directions of Squamish Nation and Yúustway’s 10 Year Community Health Plan, the Home Care Nurse Assessor promotes a culturally safe, supportive and empowering environment for Nation Members, clients and families, colleagues and partners. The Home Care Nurse Assessor preferably has experience working with First Nations communities and brings a solid understanding and application of Indigenous Cultural Safety to their practice. Reporting directly to the Manager, Home and Community Care, the Nurse Assessor is a registered nurse providing assessment, supervisory and clinical nursing skills to effectively assess and coordinate client care for the Squamish Nation Community. The incumbent provides direction and guidance to the Health Care Assistants (HCAs) and Homemakers. This position is responsible for accurate records of client assessments and manages assessment files. The incumbent is accountable for developing and maintaining relationships with external service providers for advocacy and referrals. The incumbent reviews and evaluates the satisfaction of clients regarding their home care services.
