St. Joseph's Seniors Mental Health Outpatient and Outreach Programs (SMHOPs) have a mandate for shared clinical service, learning and development and community/system development across our tertiary and regional catchments. The SMHOPs are funded from multiple sources, have various operational partnerships and a range of reporting, data collection and evaluation responsibilities. The SMHOPS are part of the continuum of inpatient and community based service of the SJHH Seniors Mental Health Services
This position exists to support program coordination which includes: evaluation, communication and reporting flow/processes related to the infrastructure primarily of the four SJHH SMH community
- based seniors mental health outreach teams however some work may be completed to support the evaluative and/or CQI needs of our inpatient SMH services. Additionally, this position will support and assist in the coordination of various operational, evaluation and/or CQI type projects within a single team, our Seniors Mental Health Service and/or at a provincial level as required
**QUALIFICATIONS**:
- Completion of a 2 year post secondary program in a relevant field e.g. business, data, health required
- 3-5 years recent intake, administrative, and/or quality/CQI experience in an academic healthcare environment or equivalent education and training
- Demonstrated knowledge and/or experience of clinical patient service work flows e.g. referral to discharge, administrative and/or quality experience in an academic healthcare environment or equivalent education and training
- Proficient in Microsoft Office software inclusive of Excel, Word, Powerpoint and Outlook
- Proficient in Dovetale administrative, reporting, patient communications functions
- Demonstrated excellent interpersonal and communication skills to work with both internal and external partners
- Demonstrated ability to develop workflows and procedures to support operations
- Skilled and efficient at addressing operational problems/challenges and facilitating processes to find a suitable solution
- Keen attention to detail and to ensure accuracy of work and reporting
- Demonstrated quality assurance analysis skills
- Demonstrated excellent organizational, time management and scheduling skills, and ability to set priorities to meet project and task deadlines required.
- Demonstrated ability to learn quickly and adapt to changing priorities required.
- Ability to successfully lead and actively participate in numerous, diverse initiatives from inception to completion organizing and prioritizing appropriately to meet needs and expectations
- Ability to work independently with little to no supervision
- Ability to generate data summaries and analyze as required
- Confidence to communicate effectively with staff from all levels of the organization and patient population
- Able to identify and assess the learning needs of staff with a demonstrated ability to teach and coach others
- Evidence of commitment to continuous education
- Demonstrated excellence in communication, team building and the ability to initiate and model positive change
- Demonstrated ability to analyze, problem solve, make recommendations for change, implement and evaluate
- Ability to maintain records of training dates, project and task completion for annual review
- Good work performance and attendance record is essential
**RESPONSIBILITIES**:
- Responsible for a general knowledge of service delivery and work flow within the Seniors Mental Health (SMH) Service to be able to assist with evaluation and data driven initiatives
- In cooperation with Manager, responsible for developing systems and infrastructure to gather various reporting, evaluation and quality data for varied uses across the community and inpatient services of Seniors Mental Health
- In cooperation with Manager, responsible for drafting relevant policy and procedure related to the above systems and/or infrastructure
- In cooperation with Manager, responsible for communicating out, coaching/training of other staff, following up and creating audit processes to be implemented and reported to Manager
- Knowledge of REDCap Software and Dovetale reporting systems which would be utilized and leveraged to assist with quality assurance/quality management and control within the Seniors Mental Health Service
- Contribute to the maintenance and improvement of systems, methods and processes within the SMH Service
