Lifemark’s Home and Community Care division, **Community Advantage Rehabilitation**(CAR) is currently seeking qualified** Rehabilitation Assistants**_ _to provide services to our clients within the community.

As Durham Region’s largest community based rehabilitative therapy provider we have been providing community rehabilitation services for adults and children since 1997. **Primary Responsibilities**:

- Deliver prescribed treatment plan as assigned by the supervising therapist in to individuals living in the community
- Deliver dynamic, safe and therapeutically beneficial home exercise programs
- Maintain regular ongoing communication with the supervising therapist regarding any changes or concerns about a client's status or treatment interventions
- Effectively communicate information to client/caregivers
- Complete and maintain current and accurate documentation related to client’s progress using EMR technology
- Understands and implements infection prevention practice

**Requirements**:

- Graduate of a recognized OTA/PTA program (required)
- Experience working within a community setting is an asset but RAs with general experience are also encouraged to apply
- Ability to work independently and as part of an inter-professional team
- Outstanding focus on safety and ethical standards
- Strong ability to build rapport with clients
- Ability to prioritize, manage self-scheduling and meet reporting deadlines
- Excellent oral, written and report writing skills
- Proficiency with computers
- Strong interpersonal and organizational skills
- Reliable transportation is required for this position
