**The Adult Protective Services Program is sponsored by the Ministry of Children, Community and Social Services and administered by Family and Children’s Services of Renfrew County.**:
Adult Protective Services Workers (APSW) assist adults with developmental disabilities with case management, advocacy and accessing services and supports to meet their needs. APSWs provide planning that is person centered and focused on the expressed goals and needs of the adult and they assist them in developing or strengthening their support networks.

**GENERAL RESPONSIBILITIES**:
**Adult Protective Services Workers (APSW) **will**:

- Support adults living on their own or with their families to address needs of heath, safety and community participation
- Provide advocacy and ensure that the rights of vulnerable adults are protected and respected
- Individuals and families will have access to a range of specialized services and support offered by Family and Children’s Services of Renfrew County as well as a wide variety of other organizations serving Renfrew County.
- Families and individuals will receive services that strive to achieve greater interdependence and participation in their communities
- Individuals will receive transitional planning to prepare for the challenges and opportunities associated with transitioning from high school to adult services and mid-life to senior services
- Ongoing collaboration with all partners in the community

**DUTIES PERFORMED BY AN** ADULT PROTECTIVE SERVICES WORKER **ON A REGULAR BASIS**:

- Upon receiving a referral, the APSW must meet the individual or family within set timelines to complete a screening and assessment to review identified goals and needs for service as expressed by the individual/ family.
- To provide individuals and families with the opportunity to plan for the services and resources which are unique to their circumstance, culture and goals
- To provide advice to individuals and families regarding their expressed needs in relation to service availability. To assist individuals and families to seek creative alternative options when service is not readily available
- Ensure that an Individual Support Plan (ISP) is developed with the wishes of the person or legal substitute decision maker being central in the design and implementation
- To ensure that plans and goal setting for all adults embrace the concept of social inclusion within our community
- Act as an advocate for the person in service or their S.D.M./guardian by representing the stated and expressed wishes of an individual to other parties. To ensure that all advocacy is documented with appropriate authorizations as required by procedures
- Ensure that the individual is aware of all foreseeable outcomes arising from advocacy initiatives and that the choice made arises from their informed decision
- Ensure that individuals at risk of imminent harm, who are unable, due to confirmed or perceived incapacity, receive case management and advocacy services for interventions to relieve the immediate crisis
- Maintain clear and accurate records and produce written and verbal reports and letters as required ensuring that all client and family information is handled in a confidential and sensitive manner, in accordance with legislation and policies
- Ensure that files meet the Quality Assurance Measures as defined by the Services and Supports to Promote the Social Inclusion of Persons with Developmental Disabilities Act (SIPPDA).
- Coordinate annual Passport contracts with APSW staff and March of Dimes and track allocations
- Work with Service Recipients and Private Passport Workers to receive Passport One invoices and ensure they meet the Passport guidelines (where we are the TPA)
- Confirm the invoices are in line with the Agency Funding Contract, Support Plan and Support Log Sheets prior to sending to admin for processing in Apricot
- Adhere to all FCSRC policies and procedures, Ministry Regulations, Standards, Directives and relevant legislation.
- Responsible for the completion of other related tasks as assigned.

**PERFORMANCE INDICATORS**
- Demonstrated use of Signs of Safety Model (principles, philosophy, tools) in planning and
