**Social Systems Navigator**

With over 30 years of highly successful experience in crafting the design, development, and delivery of a wide range of effective, client centered services, TI continues to deliver quality programs and services that directly benefit our growing urban Inuit population.

**What we are looking for**:
The Social Systems Navigator supports Inuit patients and families through their experience in the health care system while in Ottawa accessing patient services, connecting patients and their family to community cultural services and supports and implementing opportunities for enhanced cultural competencies in collaboration with stakeholders.

**What you will be doing**:

- Work closely and develop positive relationships with staff of project stakeholders
- Determine, through intake and plan of care, the needs of clients and families
- Coordinate client support, activities and services
- Develop, organize, facilitate and/or provide referrals for cultural services, as well as programming
- Direct clients to translation and/or provide translation as may be requested
- Provide clients with resources to assist in culturally appropriate services, such as pamphlets, web lists, phone numbers and transportation to events and services (bus tickets)
- Provide clients with resources and tools developed to collect evaluation of the program and services and identify barriers
- Create and provide activities for children, youth, adults and families which will be appropriate for those dealing with health issues
- Contribute to and support service coordination with all partners and stakeholders
- Ensure the program meets the mental, physical, emotional, and cultural needs of clients to receive successful treatment and follow-up care
- Submit regular data, reports and evaluation to the supervisor as required
- Abide by TI policies and procedures
- Other duties as may be required and related to the position

The social systems navigator also travels routinely within the city to different sites to support patients and their families. Frequent evenings and weekends are required.

Contract length: 12 months

**Salary**: Up to $58,000.00 per year
