**DFCC is seeking to fill the position of**: **COLLABORATIVE DAY TREATMENT PROGRAM (CDTP) CHILD AND YOUTH CARE PRACTITIONER** **Hours: 37.5 hours per week from approximately August to June annually** **Permanent position** **Start Date: as soon as possible** Do you want to be part of a dynamic Team working towards building safe, hopeful, healthy communities? Through individualized prevention, intervention and academic approaches, CDTP strives to establish a successful learning experience to promote the students’ knowledge, skills & values to enhance their life options. CDTP receives referrals throughout the school year to 2 classrooms based at DFCC’s Whitby location. The Child and Youth Care Practitioner is part of a multi-disciplinary team and provides the interventions and support to facilitate youth involvement in and benefit from formal education, including outreach and support to their family/caregivers. CDTP Child and Youth Care Practitioners are expected to meet the program’s Mission Statement of the Collaborative: "Through a multiple agency partnership and an integrated Classroom Model, provide a successful learning experience for youth that will promote their knowledge, skills and values and enhance their life options." **Summary of Responsibilities**: - Work in partnership with Durham District School Board teachers to provide an intervention/educational program, which enhances youth participation and success. - Actively communicate with caregivers and stakeholders to promote involvement in planning the youth's support and educational program. - In collaboration with the CDTP Team and the youth, develop and implement appropriate standards of behaviour and expectations and provide supervision as required. - Provide planned and ongoing individual counselling to the youth in cooperation with other counsellors and stakeholders as appropriate to assist youth in a prevention, intervention and academic setting to reach goals. - Document support and intervention plans in case management program and daily logs as directed. - Provide programming that is integrated on a group and/or on a one to one basis that: focuses on such areas as life skills, social skills, reintegration and anger management; promotes the youth’s individual strengths; and, supports the individual youth’s behavioural, emotional and social needs. - Where appropriate plan/advocate for re-entry into mainstream community schools/classes and support caregivers in this effort and/or refer to appropriate alternative educational programs. - Plan and participate in appropriate educational outings with the students. - Provide support to and assist with Nutrition Program. - May mentor Community College placement students. **Knowledge, Skills and Attributes**: - Values, promotes and embraces diversity, equity and inclusion principles and practices; - Knowledge of the Youth Criminal Justice Act, other relevant legislation and community resources pertaining to working with youth and their families; - Thorough understanding and knowledge of a range of intervention skills, issues of child abuse, mental health issues, systemic approaches, and child and youth development; and - Excellent time management techniques in order to plan own work schedule, establish priorities and to balance multiple, sometimes conflicting tasks and/or changing priorities. **Experience, Training and Education**: - Community College Diploma/Certificate or undergraduate degree. - Proven experience working with youth, including youth in conflict with the law, preferably in a classroom setting with several years’ experience in this field.
