Connecting a global network of hundreds of worldwide clubhouses, Progress Place has assisted in keeping countless members out of the hospital and provided opportunities to flourish in their communities, every day of every week for over 35 years.

Teamwork is used to blur the lines between members and staff in workplace environments both in-house and at paid employment placements with our community partners. Members can access a continuum of supports from case management, recreation to health and wellness. At the centre of Clubhouse programming is the “work-ordered-day” which mirrors a typical working day. Members and staff work in partnership to run all aspects of the organization from reporting statistics for funders, preparing almost 1,000 meals per week and developing our monthly podcast.

**Purpose of Position**:
Progress Place is looking for a Health Access Coordinator who will be situated at The Community Place Hub in the community of Weston. The coordinator will be working in partnership with over 20 partner agencies located in the Weston and Mount Dennis communities with the goal of providing seamless health access holistic services for community members. Community members will have an intake assessment at either the “Hub” or one of the partner organizations highlighting needed services, programs and resources/referrals. A warm transfer will be key in ensuring that the member/client makes the connection by having either a staff member accompany them or a peer support worker to the resource. If the connection is not satisfactory the member will be referred to resources until all possible options are explored.

MAJOR RESPONSIBILILTIES

Collaborate with stakeholders, community and Progress Place. Program support and program development will be consistent with psychosocial rehabilitation principles and the Clubhouse philosophy.

Manage administrative duties for the program.

Engage stakeholders, establish new relationships with key stakeholders and partner agencies. Attend meetings with Key stakeholders (Ontario Health and United Way), partner agencies as needed.

Coordinate the implementation and evaluation of the program from start to finish including quarterly reports.

Provide timely updates and meet project targets.

Maintain strategic diplomatic communication with other community resources as required facilitating successful connections within the agency and with outside agencies and funders.

Promote, encourage and facilitate connections to the Progress Place Clubhouse and other community programs.

Work with members of the Hub and or provide back up support to staff team of the Hub as needed and decided on with Hub Coordinator.

Participate in the evaluation of the program and its goals and objectives.

Perform other duties as may be assigned by the Manager of Support Services/ Clubhouse Manager and or Executive Director.

QUALIFICATIONS, SKILLS AND KNOWLEDGE

Demonstrate an understanding and knowledge of how to work effectively with people who have had difficulty in accessing community resources.

One-year experience in the Human Services Field, and a minimum of six months working experience with persons with a psychiatric disability.

A degree in a related field or the equivalent combination of education and work experience.

Able to work comfortably with technology and willingness to learn new technology platforms as needed.

Able to work in person and virtually.

Previous supervisory experience and team building an asset.

The Health Access Coordinator must exhibit sound judgement, tact and diplomacy; be consistent in all interaction with other staff, members, community partners and stakeholders; demonstrate cooperation with team members; show an understanding of how to work effectively with psychiatrically disabled adults; and at all times respect the confidentiality of program information.

WORKING CONDITIONS

Duties require flexible scheduling working days, evenings, weekends and holidays and involve activities, which are done on a routine basis: use PC screens, walk up and down flights of stairs, bend, stretch, lift, carry, stoop, push, reach above shoulder, and reach below shoulder.
