**POSITION**
**ADOLESCENT CENTRE WORKER**
- PARC is a non-residential preparation for independence program serving youth ages 15 to 29 years old, in and from the care of the Children’s Aid Society of Toronto (CAST), Catholic Children’s Aid Society (CCAS), and the Jewish Family and Children’s Services (JF&CS). PARC strives to provide youth in and from care access to resources in a safe, supportive community in a manner which allows them to feel in control of their lives.
The staff person will join a multi-service team who realize, in concert with the program’s young people, the aims and objectives of the Centre.
**MAJOR RESPONSIBILITIES**Participates in screening youth referrals to the Centre at the intake stage.
-Works with youth and caregivers in assessing needs of youth referred.
-Co-develops a client centred plan of service with the youth referred. Works closely with youth and caregivers to establish methods of undertaking service plan.
-Implements the plan together with the youth to effect change and growth. Again together with youth, in collaboration with other caregivers, evaluates progress and re-contracts with youth for new service.
-Participates in relevant case related conferences to propose service recommendations, share pertinent case activity information and maintain, modify or terminate service involvement.
-Provides goal-specific service delivery through a variety of modalities and techniques, which include individual and group interventions, both in the community and/or in a programme setting.
-Recommends and assists the Child Welfare Worker/Case manager in referring the client to community services and agencies; to meet new and on-going needs in developing independent living skills.
-Works with existing community resources in order to advocate for clients’ needs for independence.
-Provides crisis response where child & youth interventions are determined to be appropriate to the need and recommends continued service if necessary and warranted.
-Prepares and completes all program written requirements within stated time frames and according to agency policy.
-Participates in on-going programme planning and service delivery for the entire Centre.
-Supervises child & youth students or students in a related discipline, as required.
-Facilitates the integration of lay volunteers into service operations to enrich services offered, and may provide the supervision of volunteers as determined and needed.
-As assigned, develops and implements programs in a variety of service areas, may negotiate and writes funding proposals and project reports to funding agencies.
-Outreach to Youth, Child Welfare Workers and community agencies and share information about the various programs developed and facilitated at the Centre.
-Interviews, assesses, and assists youth to identify, plan, and achieve both short and long term goals in variety of Life skills, Employment, Mentoring, Art, Wellness, Housing and/ or training programs.
-Completes evaluation tools related to youth, by working with Children’s Aid Foundation of Canada, and participating in periodic update calls with all national program partners in ensure best practices of the program.
-Prepares case management and case note documentation and handles confidential data; ensures quality audit measures are achieved in accordance with CAST and CAFC’s standards.
-Coordinates with CAFC staff to fill program opportunities available with CAFC donors and supporters
-Provides training, guidance and ongoing support to youth and trouble shoots/coaches program participants through problems
-Works in a safe manner in accordance with the Society’s health and safety policies and procedures and all relevant legislation.
-Acts in accordance with and incorporates Society's Code of Ethics, Confidentiality, Anti-Oppression/Anti-Racism, Harassment & Discrimination policies, etc.
-Uses sound judgment in consideration of financial resources.
-Complies with Society’s financial policies and procedures.
-Performs other tasks as required.
**QUALIFICATIONS**
**Education and Experience**:
Child and Youth Care diploma (formerly Child and Youth Worker diploma),and a minimum of 3 years experience with youth similar to the PARC clientele, OR
-An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.
**Knowledge and Skills**:
Working knowledge of the child welfare and Extended Society Care ward system in Ontario, and the related psychological and social issues affecting the personal growth and opportunities of youth in care or on extended care.
