**Position Description**:
The Social Worker will work collaboratively with the Clinical Leads for the Substance Use Program and the Transgender Youth Clinic (TYC). The role involves providing social work services, including acting as a therapist for caregivers of teens with substance use disorders and providing services through in-depth psychosocial assessments and interventions for youth and caregivers in the TYC.

**Here's What You'll Get to Do**:

- Conduct psychosocial assessments working in collaboration with other members of the program team, with adolescents and caregivers of adolescents who have alcohol and substance use disorders or are affected by substance use, and who are attending individual and/or day treatment services in the Substance Use Program.
- Develop and implement treatment plans which build coping skills, improve relationships, and quality of life. Such treatment would be conducted through a variety of modalities including Acceptance and Commitment Therapy (ACT), DBT, CBT, MI, and others.
- Assume responsibility for all aspects of social work practice including issues of productivity and utilization.
- Collaborate/communicate with hospital health care teams and community agencies.
- Practice in a manner that is consistent with the philosophy of continuous quality improvement and evidence-based practice.
- Provide therapeutic support including supportive therapy and mental health counseling to adolescents and their caregivers with a variety of substance use and concurrent disorders (co-existing substance use and mental health disorders).
- Provide services through in-depth psychosocial assessments and interventions for youth and caregivers in the Transgender Youth Clinic (TYC).
- Conduct psychosocial assessments, develop and implement intervention plans, and provide brief intervention and supportive counseling.

**Here's What You'll Need**:

- A Master's degree in Social Work.
- Registration with the Ontario College of Social Workers.
- A deep understanding of the depth and complexity of issues that underlie adolescent substance use disorders and concurrent disorders.
- Knowledge and experience working with adolescents and with caregivers, particularly in day treatment programs in both group and individual formats is an asset
- Well-developed assessment and intervention skills, including therapeutic skills as well as skills in crisis management, loss and bereavement, parent work, and other mental health interventions.
- Knowledge and experience with evidence-based therapeutic modalities such as MI, CBT, DBT, or ACT with adolescents is an asset
- Well-developed strengths in advocacy and collaboration.
- Knowledge of the network of community services and resources; an understanding of health care delivery systems and the practice of social work in a secondary setting is also important.
- Ability to collaborate effectively as a member of the health care team and to take initiative and leadership when appropriate.
- Commitment to remain abreast of current literature and relevant developments in clinical practice.
- Excellent interpersonal, communication, and organizational skills.
- Demonstrated ability to utilize critical thinking and problem-solving skills.
- Demonstrated attention to detail and efficient time management.
- Demonstrated ability to function as a member of the interdisciplinary health care team. '
- Demonstrated belief in lifelong learning and evidence-based practice.
- Experience working with adolescents and parents.
- Experience working with non-binary, gender-expansive, trans, gender diverse/fluid, or non-conformity individuals is an asset.
- Demonstrates commitment and actions in advancing equity, diversity and inclusion objectives.

**Employment Type**
- Temporary, one-year contract, with modified benefits (1.0 FTE). There may be an opportunity to extend.
