**Cambridge North Dumfries Ontario Health Team Primary Care Lead**
**1 Year Contract, Renewable | 6-8 hours per week (up to a maximum of 32 hours per month)**
Healthcare in Ontario is undergoing significant transformation, and primary care leadership is critical to its success. With a strong mandate from Ontario Health for 2025/2026 focused on improving patient attachment, access, and system enablement, the role of primary care has never been more central.
In Cambridge and North Dumfries, this work is already well underway. The Cambridge North Dumfries Primary Care Network (CND PCN) was established in 2024 alongside the Cambridge North Dumfries Ontario Health Team (CND OHT) to bring together local clinicians in a coordinated and collaborative way. Building on this foundation, the CND Primary Care Network Leadership Committee was launched in 2025 to provide guidance and leadership in advancing primary care priorities within the broader health system transformation.
As outlined in recent Ministry of Health directives, primary care networks are a key mechanism for strengthening collaboration and ensuring the meaningful engagement of clinicians. These networks support clinical leadership, enable better coordination of care, and are instrumental in advancing access and attachment for patients.
The CND OHT remains committed to ensuring that primary care clinicians—both physicians and nurse practitioners—are not only engaged, but actively shaping the future of care in our region.
**About the Role**
The CND OHT Primary Care Lead will report to the CND OHT Director and be supported by the OHT staff support team. This role plays a key leadership function within the Primary Care Network and broader OHT initiatives.
- **Key Responsibilities**:_
- Co-lead monthly CND Primary Care Network Leadership Committee meetings
- Act as a liaison with the Ontario Health West Primary Care Lead, sharing provincial updates with primary care providers in Cambridge and North Dumfries
- Represent the CND OHT at local, regional, and provincial tables as the Primary Care Lead
- **Additional Responsibilities Include**:_
- Strengthening primary care involvement in CND OHT planning and implementation through a co-designed approach
- Sustaining and expanding primary care engagement (e.g., hosting sessions, supporting learning modules, facilitating readiness assessments)
- Collaborating with the CND OHT staff to advance primary care involvement in initiatives related to population health, digital health, and quality improvement

**Qualifications**:
- **Actively practicing primary care physician or nurse practitioner in Cambridge or North Dumfries**
- Deep understanding of the primary care landscape, challenges, and opportunities in the region
- Familiarity with Ontario’s health system, funding models, and policy priorities is strongly preferred
**Compensation is based on CND OHT primary care leadership remuneration rates ($165/hr). Time allocation estimated to be equivalent to 6-8 hours per week, spread throughout the week, varying based on projects or needs**
