Disability Case Manager

**Santé Circle Health**is seeking a compassionate and detail-oriented Disability Case Manager to join our team. This role involves managing short-term disability (STD), salary continuance, and workplace accommodation requests. You will handle intake, assessment, adjudication, and return-to-work planning while providing expert support to employees and stakeholders. Your empathetic approach and commitment to delivering clear and accurate decisions will be essential in this role.
**Key Responsibilities include**:
- Manage a dedicated caseload, including STD, salary continuance, and workplace accommodation requests, ensuring compliance with service-level agreements.
- Conduct intake assessments and file reviews to determine eligibility for benefits or accommodations.
- Make evidence-based decisions on claims and communicate outcomes with empathy and clarity.
- Develop and implement return-to-work plans in collaboration with employees, managers, and healthcare providers.
- Prepare and present case summaries, decision letters, and reports for internal and external stakeholders.
- Maintain comprehensive case documentation, ensuring confidentiality and accuracy.
- Provide guidance on the disability claims process and accommodation options.
**Qualifications**:
- Education and Certification: Regulated Healthcare Professional in good standing with their regulatory College. Certification in Disability Management (e.g., CDMP, CRTWC) is an asset.
- Experience: Minimum of 3 years managing occupational and non-occupational injuries/illnesses, including accommodation and return-to-work planning.
**Skills**:
- Strong understanding of disability management, medical terminology, and applicable legislation (e.g., Human Rights Code, Employment Standards Act).
- Exceptional communication skills, with the ability to handle sensitive conversations compassionately.
- Excellent verbal and written communication skills and feel comfortable speaking to people on the phone
- Demonstrate resiliency, empathy and compassion when listening and communicating with clients including claims-related decisions
- Able to use de-escalation skills with clients, if required
- Excellent analytical and decision-making skills, with attention to detail.
- Ability to initiate, self manage workload, work under time pressures and deadlines to meet service level agreements
- Proficiency in Microsoft Office Suite and case management software.

Pay: $60,000.00-$75,000.00 per year
