**WellFort is seeking Social Worker/ Clinical Counsellor to join our team**

**Social Worker/ Clinical Counsellor - Multiple roles (contract and permanent)**

The Social Worker works as a part of an interdisciplinary team functioning both independently and collaboratively with other staff in clinical, counselling and health promotion activities.

The Social Worker provides clinical, case management, advocacy, and outreach support as well as health education sessions to clients. They also create and implement education services for staff, volunteers, and community agencies as required.

The Social Worker supports the development, implementation, and evaluation of health initiatives and links with the broader health system to mobilize resources and promote overall system improvements.

**RESPONSIBILITIES**

Direct Care
- Provide short term (8-12sessions) therapeutic counselling services from assessment to termination, including crisis intervention as needed. The clinical counsellor will utilize a variety of therapeutic modalities depending on the needs of the client (e.g. CBT, DBT, EFT, IFS, narrative, solutions focused, etc.)
- Maintain strong documentation practices, consult with other members of the care team as necessary, and actively participate in clinical and peer supervision.
- Plan and facilitate group programming to meet the unique needs of clients served.

Case Management
- Participates in and/or leads regular case conferences to ensure good communication regarding the needs of the client and to promote care planning and improvements in the quality of service provided.
- Arranges and follows up with case consultation and case planning with other members of the team.
- Develops connections with service providers in other agencies and institutions in which the client is involved to facilitate comprehensive case planning.
- Consultation with other staff and health professionals as required in establishing a plan of care.
- In collaboration with other staff and/or shared care partners, monitor and evaluate client progress and recommend adjustments to treatment plans as needed.
- Maintains supportive relationships with clients and family members.
- Maintains up to date knowledge of community resources.

Advocacy and Community Development
- Provide case advocacy to ensure that clients receive appropriate community and health supports.
- Participates in community projects or WellFort activities to provide education, information and/or to develop collaborative strategies

Health Promotion & Education
- Actively participate in the planning, development, implementation, and evaluation of health promotion and education and support programs that are relevant and meaningful to the community served by WellFort, at off-site locations, and in collaboration with community partners.
- Act as a resource to WellFort staff and in partner/community organizations on specific and general issues regarding community mental health.
- Identify gaps in services and promotes the introduction of revised or new services.

Other
- Other duties and responsibilities may be assigned from time to time.

**QUALIFICATION, EXPERIENCE & SKILLS**
- A master’s degree in social work or a related field.
- Current member of the Ontario College of Social Work and Social Service Workers (OCSWSSW) as Social Worker or Registered Social Worker (RSW)
- Three to five years’ experience providing counselling, case management, and/or advocacy with marginalized and isolated individuals
- Experience in providing therapeutic counselling and support to clients
- Experience and/or openness to working with communities and individuals who face barriers accessing traditional health care institutions for reasons as such as poverty, discrimination, mental health, substance use issues, and others
- Well developed counselling skills including assessments, interviewing, goal setting and short-term focused work are essential.
- A capacity for critical analysis of the social determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.
- Excellent written communication and oral skills
- Experience working with confidential and time sensitive documents and information.
- Excellent interpersonal skills and the ability to multi-task in a fast-paced environment.
- Experience working in a value based non-profit or social services organization.
- Demonstrated ability to work independently and as a part of an interdisciplinary team in which the skills and responsibilities of each team member are recognized and respected.
- Valid Ontario Driver’s license and access to a vehicle (insurance which allows clients to travel in the Social Worker’s car is a definite asset)
- Lived experience with any of the populations is an asset
