**PURPOSE**

**RESPONSIBILITIES**
- Provide counselling and case management to individuals living with, at risk of, and affected by HIV and HCV across Peel Region
- Support clients to develop goals and provide appropriate client-centred interventions
- Provide community-based education sessions on various issues including HIV, HCV, Harm Reduction, Mental Health, and more.
- Screen/assess for self-harm/risk factors and create safety plans with clients as needed.
- Engage in individual and systemic advocacy related to needs articulated by community members
- Determine need for additional supports and make appropriate external referrals as needed.
- Build and maintain strong relationships with community partners.
- Maintain accurate and up to date client records in the EMR.
- Ensure comprehensive treatment through ongoing consultation and collaboration with team members.
- Actively participate internal meetings as well as external community tables and committees

**QUALIFICATIONS, EXPERIENCE & SKILLS**
- Master’s degree in Social Work or a related field.
- Member in good standing with Ontario College of Social Workers and Social Service Workers and licensed to practice the controlled act of psychotherapy.
- Strong experience with, understanding of, and commitment to harm reduction principles.
- Demonstrated commitment to, and experience working from an anti-racist, anti-oppressive, and trauma informed framework.
- Experience working with people experiencing homelessness, people who use drugs, people who are incarcerated or otherwise involved in the criminal justice system, youth, and Indigenous community members
- Lived/living experience with any of the populations served by Bloom Clinic is an asset
- Experience working with communities and individuals who face barriers accessing tradition health care institutions such as poverty, discrimination, mental health, and substance use issues.
- An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.
- Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.
- Experience working with confidential and time sensitive documents and information.
- Excellent interpersonal skills and the ability to multitask in a fast-paced environment.
- Experience working in a value based non-profit or social services organization.
- Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.
- Ability to speak languages relevant to the residents of the organization’s community is an asset.
- Valid drivers G drivers licence and access to a reliable vehicle.
