AFCC is the proud recipient of the 2024 Calgary Awards, Community Advocate Organization of the Year Award. ***: The Housing Case Manager is responsible for direct implementation of AFCC Housing philosophy and policies as they pertain to AFCC Homeless Initiative in compliance with AFCC Mission, Vision and Values as well as our goals, objectives, policies and procedures. **DUTIES**: - Assist homeless Indigenous people to complete client information/intake assessment. - Assist, support, educate and direct Indigenous homeless people in organizing accommodation search. - Coordinate access for start-up kits and furniture for Indigenous homeless people moving into their own accommodations. - Provide administrative support as required to assist Indigenous homeless people with setting up housing, able to assist with research on homelessness. - Provide administrative support and referrals to clients with addictions, mental health and behavior management issues. - Provide education to Indigenous homeless people with regard to the "Residential Tenancy Act". - Provide holistic and comprehensive case management services to all clients including intake assessment, benefit assessment, goal setting, long-term case development, weekly case plan development, progress monitoring, individual money management, tenant education, advocacy and referrals. - Maintain a caseload of 10-12 individuals. - Meet with clients weekly or bi-weekly for up to 12 -36 months. For the first 6 months of a client's participation in program, at least 80% of follow-up meetings held in-person. - Connect individuals with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral. - Input accurate and complete date for all contacts with clients into agency database. - Ensure that all documents submitted on behalf of a client are valid. - Contact landlords to validate eviction notes. - Other duties as required. **QUALIFICATIONS**: - Post secondary education in a relevant social service practice or social science including but not limited to child and youth care, social work, addictions, psychology or sociology. - Must have prior knowledge of Indigenous Cultural practices and experience with Indigenous ceremonial practices. - Extensive knowledge of current housing options. - Extensive knowledge of the Indigenous Community Resources. - Effective interpersonal and verbal/written communications skills. - Experience in residential care settings. - Experience in outreach services to Indigenous homeless people and the housing first model of care. - Strong computer skills and experience working with the HMIS system. - Valid Alberta Driver’s license, reliable vehicle that is available for work use on a daily basis, clean driving abstract with proof of liability insurance - Police Info Check (PIC) & Intervention Record Check. This is a full-time in office position with 40 hours per week, Monday to Friday 8:00 am - 4:00 pm.
