**Position Overview**
We are seeking a passionate and dedicated Affordable Housing Case Manager to join the Housing team at the INN. The Affordable Housing Case Manager will work with 31 families residing in the INN’s two affordable housing locations. They will provide Case Management, supporting families in attaining stability, sustaining their permanent housing, and achieving their goals to move forward in their lives. The Case Manager also serves as the primary liaison between the INN and its housing partner, HomeSpace, fostering collaborative relationships and providing proactive support to address tenancy-related challenges.
This is a full-time position, Mon-Fri 9 am - 5 pm. Some evening/weekend hours may be required, dependent upon individual client family needs.
**What You’ll Do**
- Support participant families as they initially enter the Affordable Housing program - working with Shelter Case Managers to facilitate a warm handover and supporting with lease signing and meeting with landlord.
- Support participants in identifying needs, barriers and challenges - and setting and attaining goals to address them.
- Meet with families regularly to provide support and monitor progress.
- Provide ongoing advocacy & resource navigation support to address barriers and assist families in accessing culturally relevant community resources, employment services, mental health support, health care and financial assistance.
- Act as liaise and main point of contact with the INN’s Affordable Housing partner landlord, HomeSpace - maintaining a strong professional relationship and supporting participants proactively with all tenancy related concerns.
- Maintain accurate, timely, and confidential documentation of family interactions, case notes, intake and assessment information, safety concerns and records of support and service referrals using HOMES and HMIS databases, physical participant file.
- Travel between the INN’s main site, and our satellite Affordable Housing site.
This position description reflects the general duties of the role but is not intended to include all duties that may be inherent in the position. The Affordable Housing Case Manager may be asked to perform other duties as required or assigned.
**What You Bring to Our Mission**
- Completion of a diploma or degree program related to social work, family case management, or human services is preferred but a combination of education, experience and training will be considered.
- 2 years experience in family case management, family support and/or crisis support.
- Experience working with vulnerable populations experiencing homelessness.
- Knowledge of trauma-informed, client-centered care and conflict resolution skills.
- Knowledge and understanding of issues related to homelessness and its impact on families
- Computer literacy skills are required, including the ability to use MS Office 365 and Teams.
- Excellent communication, problem-solving, and teamwork skills.
- Able to communicate effectively both verbally and in writing in the English language.
- Able to work effectively and safely without direct support or supervision in the community.
- Current First Aid/CPR, Suicide Intervention (ASIST) and Crisis De-escalation training is required.
- Police check and Child Intervention check are required for employment.
