Job Summary

The Team Lead at the Transition House is responsible for providing day-to-day leadership and oversight to ensure the delivery of quality services and effective case management for residents. Reporting to the Transition House Manager, the Team Lead works closely with all Transition House staff and other Crossroads employees to promote best practices and trauma-informed care within the shelter. This role includes shift coordination, staff support, case consultation, and administrative oversight and support.

**Responsibilities**:

- Lead shift exchanges, ensuring clear communication between outgoing and incoming staff.
- Complete and assign daily work assignments for interveners based on resident/client needs and operational priorities.
- Ensure that Crisis Interveners are supporting clients/residents using best practices, including trauma-informed, anti-oppressive, and client-centred approaches.
- Provide support and direction to Crisis Interveners, including informal coaching and guidance during shifts.
- Ensure staff have the necessary tools and resources to achieve the best outcomes for residents; work with the Transition House Manager to fill gaps.
- Collaborate with Management in identifying specific training or skill development to enhance service delivery and professional growth.
- Oversee the implementation of individualized case management plans, ensuring consistency and quality of service.
- Meet weekly with the Transition House Manager to review and discuss client files, progress, and concerns.
- Regularly consult with other program leads, such as child and youth and housing, to ensure integrated, holistic support for residents.
- Promptly consult with management on any case management concerns or complex client issues.
- Ensure that all critical client/resident information is documented accurately, confidentially, and in a timely manner.
- Monitor documentation practices to maintain compliance with agency policies and professional standards.
- Support staff in understanding documentation requirements and provide reminders or direction as needed.
- Support an environment of open, respectful, and professional communication among staff and program partners.
- Collaborate with other program leaders to ensure alignment of services and case planning.

**Qualifications**:

- Education: Diploma or Degree in Social Work, Psychology, Human Services or a related field; relevant or lived experience.
- Experience: Minimum 2 years in crisis intervention or residential work, with demonstrated leadership ability.
- Knowledge: Strong understanding of gender-based violence, trauma-informed care, and anti-oppressive practice.
- Communication: Strong verbal and written communication skills in English (French bilingualism preferred)
- Data Management: Experience in case notes, preparing reports, collecting statistics and handling confidential client information.
- Flexibility: Ability to work occasional evenings or weekends as needed.
- Other Requirements:

- Ability to participate in an on-call rotation
- Valid driver’s license
- Satisfactory vulnerable sector and criminal record check.
