Reporting to the Director - Outpatient Rehabilitative Care, the Manager provides leadership to improve access, transitions, promote adoption of best practices, and system monitoring and evaluation for rehabilitation care in North West Ontario that advances the regional and provincial priorities consistent with the mission, vision and values of St. Joseph's Care Group (SJCG). The Manager must be able to lead self, engage others, achieve results, develop coalitions, and conduct systems transformation to support rehabilitation across the Northwest. The Manager uses a collaborative approach, inclusive of people with lived experience and family partners, to plan budgets and service delivery, manage fiscal and human resources, and achieve performance targets for quality client outcomes.

Accountable to SJCG leadership and guided by Ontario Health North West priorities, the Manager plays a key role in building relationships with local, regional, and provincial health care partners to advance integration of rehabilitation service delivery, ensuring access across the continuum of care and smooth transitions between levels of care. Working within the North West region health structure, the manager collaborates with the four Ontario Health Teams (OHT) and non-signatory First Nations communities/agencies, including, Federal Indigenous agencies (i.e. ISC in local system planning) and the Regional Specialized Services Network (RSSN) in regional planning for specialized rehabilitation service. The Manager facilitates an environment of collaboration and cooperation with leaders, physicians, staff, partners, and clients and families demonstrating effective communication and engagement to improve the access to and quality of rehabilitation care received by residents in the North West.

**Qualifications**:
**Education/Experience**:

- Bachelor's degree in a clinically-related discipline, with a preference for Occupational Therapy or Physiotherapy
- Registered health care professional in Ontario
- Minimum five years' progressive leadership experience with the ability to manage complex projects involving multiple agencies, budgets and partners
- Knowledge and/or experience of rehabilitative care services and service delivery models (i.e. regional, provincial and national policy/funding issues)
- Knowledge of the current and changing dynamics of the health care system, related support services, and the roles of health professionals in rehabilitative care
- Demonstrated knowledge of community resources and community needs
- Experience developing, implementing and sustaining network/partnerships
- Strategic planning and project management experience, including demonstrated business skills, as well as experience in program and system evaluation

**Skills/Abilities**:

- Ability to communicate effectively and concisely, both orally and in writing
- Proven ability to establish and maintain co-operative and collaborative relations with private and public officials, co-workers, and other partners
- Strong understanding of Indigenous cultures, traditions, and health disparities, particularly within the context of northern communities
- Strong project/time management, analytical/critical thinking, problem-solving, and decision-making skills
- Ability to work independently, utilizing discretion and sound judgment
- Strong leadership skills, including the ability to motivate and recognize others; proven ability to share skills and knowledge with others
- Demonstrated knowledge and commitment to the First Nations principles of data ownership, control, access, and possession (OCAP)
- Proven acumen in business and financial management
- Ability to identify, understand, and meet/exceed the requirements of internal and external customers
- Proven conflict management abilities and mediation skills
- Committed to ongoing professional and personal development
- Must be competent within the meaning of the Occupational Health & Safety Act
- This position shall be conditional upon a satisfactory Criminal Record and Judicial Matters Check (CRJMC) where indicated, to ensure the absence of relevant criminal convictions.
