**The main purpose of this position**
The Social Worker, Mobile Health Team is responsible for providing clinical assessment, counselling, therapeutic interventions, and systems navigation to support individuals who are unhoused and/or experiencing mental health and substance use concerns across Halton.
Working primarily in the community, this role brings a professional MSW lens to service delivery—integrating trauma-informed care, harm reduction, and advocacy to address immediate needs and long-term wellness goals.
The Social Worker is a core member of an Interprofessional Primary Care Team, collaborating with physicians, nurse practitioners, registered nurses, peer workers, and community partners.
**The main purpose of the Mobile Health Team**
The Mobile Health Team provides community-based primary care and clinical support across the Halton region. The team focuses on people who are unattached to primary care, unhoused, or experiencing complex health needs, including mental health and substance use.
Working in shelters, supportive housing, and through street-level outreach, the team meets people where they are. Members include a physician, nurse practitioners, registered nurses, social workers, peer support workers, and program support staff.
**Responsibilities**
- Client-Focused Practice_
- Provide clinical assessment, short-term counselling, and evidence-informed therapeutic interventions to individuals 16+.
- Develop, implement, and monitor individualized treatment and recovery plans in collaboration with clients.
- Deliver crisis stabilization strategies, including suicide intervention, substance use support, and de-escalation.
- Deliver evidence-informed psychotherapy groups and/or other relevant group-based therapies
- Facilitate access to health care, housing, and community resources through advocacy and system navigation.
- Build trusting, client-directed relationships using trauma-informed, harm reduction, and culturally safe approaches.
- Collaboration & System Engagement _
- Work in close partnership with primary care, addiction medicine, and community providers.
- Act as a clinical resource for the Mobile Health Team, providing consultation on complex cases and trauma-informed approaches.
- Represent Support House in community networks, committees, and partnerships.
- Promote “every door is the right door” by ensuring warm, effective referrals.

- Maintain practice in alignment with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) Code of Ethics and Standards.
- Engage in ongoing training, supervision, and evidence-informed learning.
- Participate in debriefs, annual reviews, and relevant Communities of Practice.

- Maintain self-awareness and reflective practice to support professional effectiveness.
- Identify and communicate work-related needs for accommodation and support.
- Uphold healthy boundaries and navigate dual relationships ethically.

**Knowledge and skills necessary to be successful in this role**
- Master of Social Work (MSW) registered and in good standing with OCSWSSW.
- Minimum 5 years’ experience supporting individuals who are unhoused and/or living with mental health and substance use concerns.
- Advanced expertise in trauma-informed care, harm reduction, Housing First, and crisis intervention.
- Skilled in clinical assessment, counselling, therapeutic intervention, treatment planning, and case consultation.
- Knowledge of developmental trauma, acquired brain injury, and chronic homelessness impacts.
- Experience with psychotherapy program facilitation and development (asset).
- Familiarity with tools such as LOCUS, OCAN, or VI-SPDAT (asset).
- Broad knowledge of community resources and demonstrated advocacy skills.
- Strong documentation and electronic health record experience.
- Excellent communication—both written and verbal—with individuals and teams.
- Proficiency in Microsoft Office and automated assessment tools.
- Valid driver’s license and access to a vehicle.
- Current First Aid & CPR; Naloxone and ASIST training (asset).
